Cargo, cargo units and cargo transport units carried on or under deck shall be so loaded, stowed and secured as to prevent as far as practicable, throughout the voyage, damage or hazard to the ship and the persons on board, and loss of cargo overboard. PSCO observed cargo on deck (storage containers, acetylene and oxygen cylinder crates) that are not secured in accordance with the vessel cargo securing manual. In addition, several lashings (turnbuckles) are also bent, with one completely broken. 74 SOLAS (14) VI/5.1

Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. Unless secured by closely spaced bolts, the covers shall be permanently attached. PSCO observed several watertight hatch covers leading to ballast tanks with missing nuts and broken securing posts.
