Such monitoring equipment shall also provide an alarm facility which is activated by detection of over pressure or under pressure within a tank. PSCO observed audible and visual alarms for all cargo tanks disabled in CCR. Additionally cargo tank 11CP had no readings for pressure ullage and temperature. After trouble shooting by ships crew no tanks have read outs. 74SOLAS(20)VII/10.1; IBC 8.3.3.

Cargo tanks shall be fitted with one of the following gauging devices .3 Closed device - which penetrates the tank but which is part of a closed system and keeps tank content from being released. PSCO observed inconsistent gauging readings across stowage plan, cargo tank monitoring system, and cargo control workstation, variations were between 3 and 9% across all tanks. 74SOLAS(20) VII/10.1; IBC 13.1.1

Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. In addition, it shall be provided with alarm arrangements to indicate when this level cannot be maintained. The system shall also be provided with arrangements to ensure that any discharge of oily mixtures is automatically stopped when the oil content of the effluent exceeds 15 parts per million. In considering the design of such equipment and approvals, the Administration shall have regard to the specification recommended by the Organization. PSCO observed inoperable recording device for the vessel's OWS. The crew did provide a flag dispensation but were not properly following the protocols required by the dispensation. MARPOL ANNEX I/14.7

Any failure of the oil filtering equipment shall be recorded in the Oil Record Book Part I. PSCO observed no entry in Oil Record Book Part I pertaining to the OWS recorder failure on 16APR2025. Additionally PSCO observed multiple entries attesting to satisfactory test of OWS and monitoring equipment after reporting inop recording device. MARPOL ANNEX I/17.5

A chemical tanker shall comply with the requirements of the International Bulk Chemical Code...Electrical installations shall be such as to minimize the risk of fire and explosion from flammable products. PSCO observed tank radar ground wires missing and also fitted with improper wire size. Ship's crew had replaced original ground wires with improperly sized replacements. 74SOLAS(20) VII/10.1; IBC 10.1.2.1

The system shall be capable of maintaining the atmosphere in any part of any cargo tank with an oxygen content not exceeding 8% by volume and at a positive pressure at all times in port and at sea except when it is necessary for such a tank to be gas-free. PSCO observed that the inert gas generator was not capable of maintaining an oxygen content below 8%. 74SOLAS(2001) II-2/62.2.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. PSCO observed leaking pneumatic line on valve controller for IG Blower A. Leak was vaporizing substance around valve controller. 74SOLAS(20) I/11(a)

In meeting these requirements, Company shall ensure that, .1 Inspections are held at appropriate intervals, .2 any nonconformities are reported, with possible cause if known, .3 Appropriate corrective action is taken. The technical and operational nature of the deficiencies 1-7 provide objective evidence of a failure or lack of effectiveness, of the implementation of the ISM Code. An internal ISM Audit by the company with a report of findings satisfactory to the OCMI must be submitted within 90days. 74SOLAS(20) IX/13.1, ISM Code 10.2

Each vessel vapor connection flange face must have a permanent stud projecting outwards that has a 12.7 mm diameter and is at least 25.4 mm in length. PSCO observed #3 Vapor connection on port side with a less than 1" stud. 46CFR39.2001(j)

A chemical tanker shall comply with the requirements of the international bulk chemical code and shall, in addition to the requirements of regulation I/8, I/9, and I/10 as applicable, of surveyed and certified as provided for in that condition. 74 SOLAS 20 VII/10.1. Suitably marked decontamination showers and eyewash shall be available on deck in convenient locations. The showers and eye wash shall be operational in all ambient temperatures. PSCO was not able to observe operable showers or eyewash stations due to frozen lines. IBC CODE 14.3.4; Rectify Prior to Departure; to the satisfaction of the Administration and the Coast Guard.

Every steam pipe and every fitting connected thereto through which steam may pass shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. The Inert Gas System shall be capable of inerting each tank to a level at which combustion cannot be supported. Vessel's crew could not demostrate proper operation of the Inert Gas Generator. 74SOLAS(2001)II-2/62.2.1

After completion of any NLS cargo operation, the operation shall be promptly recorded in the cargo record book. The February 9th loading of Urea Solution and Urea Ammonium Nitrate Solution were not recorded in the cargo record book. MARPOL II/15.2

The owner of a foreign flag vessel wishing to have the Coast Guard conduct a Certificate of Compliance (COC) examination must proceed as follows: (a) Notify the Officer in Charge, Marine Inspection of the port where the vessel is to be inspected at least 7 days before the vessel arrives and arrange the exact time and other details of the examination.

Vessel current CSR #10 Amendments does not accurately reflect the vessel's Document of Compliance (ISM-DOC) audit authority. Vessel's DOC is issued by Liberia with an audit conducted by DNV-GL. Current CSR reflects DOC was issued by Liberia with an audit by Lloyds Register. SOLAS (2014) CH XI-1/Reg 5.3.12

1899 - The IGS shall be capable of delivering IG with O2 content of not more than 5% by volume in the IG supply main to the cargo tanks at any required rate of flow. During operational test of the IGG, the O2 analyzer displayed 0.0% due to the high concentration of soot after the scrubber. Ship could not prove inert gas was not more than 5% by volume. 74 SOLAS (01) II-2/62.3.2 17AC

1899 - Special consideration shall be given to the design and location of scrubber and blowers with relevant piping and fittings in order to prevent flue gas leakages into enclosed spaces. IG main line after gas regulating valve IWO start air receivers is wasted and would not prevent flue gas from leaking into the engine room. 74 SOLAS II-2/62.8.1 17AC

1840 - A tank overflow alarm must be audible and visual in that part of the deck where the containment systems are located and at the point where cargo loading is controlled on the tankship.
