The owner or managing operator shall notify the cognizant OCMI as far in advance as possible whenever a vessel is to undergo repairs or alterations affecting the safety of the vessel, together with the nature of any repairs or alterations contemplated. Hull repairs or alternations that affect the safety of the vessel include but are not limited to the replacement, repair, or refastening of planking, plating, or structural members including the repairs of cracks. Attending Marine Inspector identified an area on the outer chine that is cracked and starting to show signs of wastage. Provide this office with a repair proposal to include how you will repair/replace and requested timeline to make said repairs. 46 CFR 176.665(a)

33 CFR 83.30(b)- A vessel less than 50 meters in length at anchor shall exhibit where it can be seen an all around light instead of the lights prescribed in parqgraph (a) of this rule. Vessels rear halh of the all around light was out. Vessel limited to daytime operations until repaired.

Each ring life buoy on a vessel must: (1) Be approved in accordance with § 160.050 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.150. The plug was missing from the port ring life buoy. 46CFR180.70(b)(1)

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessel did not have approved alcohol program swabs on board. 46CFR4.06-15(a)(1)

A life float required, must be approved under approval series 160.027 or other standard specified by the Commandant. The life float currently onboard is missing the data plate which indicates the approved series. The life float must be replaced with an approved float prior to 01 November (cold water requirements).

Vessel required to conduct sea trials to check for leaks and weepage after completing Dry-dock/ISE.

Replace port valve.

Renew bolts between aft section of keel and timberhorn as original.

Provide repair proposal for wasted stern block and wasted frame in engineroom to good wood.

Repair hole in plank aft of Steering rudder post as marked.

Repair transverse floor beam in steering compt. Rotted on port side at hull.

Remove Stbd rub rail and make permanent repairs to temp repairs approved in 2010, in way of accomodation space.

Remove worm shoe damaged during recent grounding.

Noted fasteners on port side wasted in area around through hull fitting which is main ground. Replace fasteners on Chine, keelson and two longitudinal frames between fasteners P9-P10, P7-P8 and P11-P13. As marked.

Provide proof that the fasteners on the port side are bedded in good wood,(planks have at least one other set of fastener holes in same area).

Provide testing results to alleviate stray current from main grounded through hull fitting. Second fastener pull with same galvanic corrosion problem area.

Propane tank distribution line kinked and main feed line to stove dry rotted. Replace with approved hoses.

Provide spacers between Lifefloats.

Provide new SOLAS retroreflective tape for ring buoys, Life Floats, and Life jackets as noted.

Fireboy cyliner out of Hydro. Re-hydro or replace. Fire boy tested satisfactory and was inspected , 18FEB2011, by Fireline Inc.

While underway noted water intrusion on Port side of Bow compartment as well as localized dry rot, marked items. Repair at next drydock scheduled for the week of 28Feb2011.

Provide onboard a FCC Marine Radio Operators Permit. Only restricted permit on board.

Replace 04 Adult Type I PFD's for a total of 43 onboard. 04 PDF's rejected by inspector.

Replace line leading from ring lifebuoy to light. Line is wasted.

Repair deck area at stern. Isolated areas where rot is present. Repair at upcoming dry dock.

During hull inspection found a punky area on the hull of the vessel above the water line on the stbd side. Effect repairs to the satisfaction of attending marine inspector.

Prove proper operation pre-eng fire boy system alarm. Tested fireboy, alarm did not sound.

Replace grab lines on STBD Life ring buoy.

Provide proof of FCC operators permit.

Vessel is restricted to carrying a maximum of 34 passengers, 36 total persons until a Coast Guard witnessed simplified stability test is conducted.
