An airlock should only be permitted between a gas-dangerous zone on the open weather deck and a gas-safe space and should consist of two steel doors substantially gastight spaced at least 1.5m but not more than 2.5m apart. PSCO observed inner door of electric motor room airlock to contain an aperture removing its ability to remain gas tight.

PSCO observed BWM Plan was missing section .5 and that appendix 1 contained inaccurate vessel information.
