For vessels carrying propylene oxide a water spray system shall be fitted to sufficiently blanket the loading manifold, exposed piping, and tank domes.
PSCO observed wasting on deck spray supply piping near port manifold resulting in a 2cm hole. Vessel is restricted from carrying propylene oxide until repaired.
Condition: Improper/Lack of Maintenance
Action required: 40 - Rectify deficiencies prior to next US port after sailing foreign
