When flammable or combustible liquid are being transported on portable tanks onboard the vessel, additional fire extinguishing equipment must be carried onboard, or a fire hose fitted with an approved portable mechanical foam nozzle with pick-up tube (eductor) and two 19 L (5 gallon) cans of foam liquid concentrate. Eductor was onboard but was inoperable and painted so the numbers were illegible and the foam liquid concentrate was not labeled to show what was in the containers or how much. The eductor and foam concentrate must be replaced.

Multiple remotely operated fuel shutoff valves failed to operate and multiple air leaks were found through out the system causing failure.

Relief valves are not required to have huddling chambers for other than steam service. During inspection of the air receivers MI observed MAWP of 200 psi. Relief valve at 185psI with the 10%+/- has the ability to exceed the MAWP. Replace relief valve to satisfaction of attending MI.
