Objective evidence indicates a failure to maintain appropriate work/rest hours, critical safety equipment on the main engine, and safe working conditions for crewmembers. All persons who are assigned duty officer in charge of a watch or as a rating forming part of a watch and those whose duties involved designated safety, prevention of pollution and security duties shall be provided with a rest period of not less than a minimum of 10 hours of rest in any 24-hour period. Upon review of “Overtime / Rest Reports”, the Port State Control Officer (PSCO) determined that eight (8) crew members in the engineering department were not provided the required rest in a 5 day sample size. During port call, a crew member was also significantly injured after an observed 18 hours without rest. An audit of the work rest hours is directly requested as part of the above audit. Internal combustion engines of 2,250 kW and above or having cylinders of more than 300 mm bore shall be provided with crankcase oil mist detectors or engine bearing temperature monitors or equivalent devices. PSCO observed that the oil mist detection system for the main engine was inoperable. The system's alarm panel indicated a fault, and the crew was unable to demonstrate its operational readiness or provide objective evidence how long the system had been inoperable. Associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to a persons on board. PSCO observed that a quick-closing valve for a sludge tank was obstructed by a pair of vice-grips. The tool was clamped onto the valve mechanism in such a way that it physically prevented the valve from being shut, rendering the remote emergency closing function completely inoperable. Every steam pipe and every fitting connected thereto through which steam may pass, shall be so designed, constructed and installed as to withstand the maximum working stresses to which it may be subjected. PSCO observed a steam pipe valve actively leaking in the purifier room. The continuous leak of high-temperature steam created a hazardous condition for any personnel entering or working in the space, posing a significant risk of severe burns. The international shore connection flange shall have a flat face on one side and, on the other side, it shall be permanently attached to a coupling that will fit the ship's hydrant and hose. PSCO observed that the gaskets for the international shore connection were deteriorated to such an extent that a proper seal could not be achieved and condition was last verified on 19DEC25. This condition renders the connection unusable, preventing the vessel from connecting to a shoreside water supply for firefighting purposes. SOLAS (09) II-2/10.2.1.7.1 FSS Code 2.2.2.1 Code: 16AC

74 solas (01) II-1/29.12.3 15AC A fixed storage tank having sufficient capacity to recharge at least one power actuating system including the reservoir, where the main steering gear is required to be power-operated. The storage tank shall be connected by piping in such a manner that permanently connected the hydraulic systems can be readily recharged from a position within the steering gear compartment and shall be provided with a contents gauge. PSCO observed steering gear room fixed storage tank gauge leaks when testing level. Unable to ascertain accurate measurement of contents in spare tank.
