"This fire detection system shall be so designed and the detectors so positioned as to detect rapidly the onset of fire in any part of those spaces..." PSCO observed detectors in Zone 14 of periodically unattended machinery space could not activate. 74 SOLAS II-2/14.2 (1997)

The function of the detection system shall be periodically, tested to the satisfaction of the administration by means of equipment producing... smoke or aerosol particles having the appropriate range of density or particle size... with incipient fires to which the detector is designed to respond. All detectors shall be of a type such that they can be tested for correct operation and restored to normal surveillance without the renewal of any component. Vessel demonstrated proper operation smoke detectors utilizing an improvised plastic jug that involved a crew member standing on surrounding equipment to reach overhead. To the satisfaction of the Administration, vessel will obtain appropriate smoke detector testing equipment. 74 SOLAS (97) II-2 / REG 13.1.13

The safety management system should include procedures ensuring that non-conformities, accidents and hazardous situations are reported to the company. PSCO discovered that the master failed to notify the company/COTP, without delay, regarding the fuel oil leak in cargo hold number 2.The technical and/or operational nature of deficiencies 02 and 03 provide objective evidence of a serious failure, or lack of effectiveness, of the implementation of the ISM Code. A safety management audit carried out by the Administration or the RO, with a corresponding report of findings satisfactory to the OCMI must be submitted prior to clearance of this deficiency and release from detention. 74SOLAS(2024) Ch IX/3.1 ISM Code 9.1

Each operation described in paragraph 2 of this regulation shall be fully recorded without delay in the Oil Record Book Part I. Transfer of fuel from the number 2 starboard fuel oil tank to the number 5 starboard fuel oil tank was falsely logged in the Oil Record Book Part I. MARPOL Annex I/17.4

All ships engaged on international voyages shall keep on board a record of navigational activities and incidents which are of importance to safety of navigation. The discovery of the fuel oil leak was falsely logged in the official logbook. 74SOLAS(24)V/28.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The vessel reported a fuel oil leak coming from the No. 2 starboard fuel oil tank into cargo hold No. 2. PSCO was provided objective evidence from the RO (RINA) of significant wastage/pitting of the number 2 starboard fuel oil tank. 74SOLAS(24)I/11(a)

Any required fixed fire detection and fire alarm system with manually operated call points shall be capable of immediate operation at all times. PSCO observed 01 In-operable smoke detector in E/R (02-18). 74SOLAS(97)II-2/13.1.1

Doors fitted in boundary bulkheads of machinery spaces of category A shall be reasonably gastight and self-closing. PSCO observed 03 doors in the engine room that did not latch. 74SOLAS(97)II-2/47.1

The minimum fire integrity of bulkheads and decks shall be a prescribed in tables 44.1 and 44.2. PSCO observed stuffing tube missing fire proof insulation between electrical space and ECR. 74SOLAS(97)II-2/44.1

The machinery, associated piping and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any dangers to persons onboard... PSCO observed fuel leak on number 11 cylinder. 74 SOLAS (01) II-1/26.1
