The condition of the ship and its equipment should be maintained to conform with the provisions of the Code to ensure that the ship will remain fit to proceed to sea without danger to the ship or persons on board or without presenting unreasonable threat of harm to the marine environment. PSCO observed 04 cargo compressors, each having a cargo leak. Vessel is restricted cargo operations prior to class attestation of proper repairs.

EMERGENCY GENERATOR SETS ARRANGED TO BE AUTOMATICALLY STARTED SHALL BE EQUIPPED WITH STARTING DEVICES APPROVED BY THE ADMINISTRATION WITH A STORED EMERGENCY CAPABILITY OF AT LEAST 3 CONSECUTIVE STARTS. THE PRIMARY SOURCE OF POWER FOR THE EMERGENCY GENERATOR WAS INOPERABLE WHEN CONDUCTING OPERATIONAL TESTS.

The Machinery, boilers and other pressure vessels, associated piping systems and fitting shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to... other hazards. The vessel experienced an equipment failure while transiting the navigable waters of the United States which adversely affected its maneuverability and posed a hazard to vessels and infrastructure along the Houston Ship Channel. 1410-Propulsion main engine-Means shall be provided to control leaks of flammable liquids. PSCO observed excessive fuel leaks on high pressure fuel pumps 1-4 on main engine. SOLAS 74 II-2/4.1.1 60a

1199- Other (Cargo) – Airlock doors should be self closing and without any hold back arrangement. PSCO observed the entry door to the motor room airlock does not close completely. Repair door prior to loading cargo IGC CODE 3.6.2 a

1199- Other (Cargo) – Cargo tanks fitted with pressure relief valves which can be set at more than one set pressure IAW 8.2.6 high pressure alarms should be provided for each set pressure. PSCO was not able to observe correct calibration of alarms due to inop test equipment. IGC CODE 13.4.1 10c
