The company should identify equipment and technical systems the sudden operational failure of which may result in hazardous situations. The vessel is equipped with fixed hydrocarbon gas detection for the ballast tanks. The system showed calibration fault and was not able to complete testing as required by the manual. This was not reported to the company.

Resolution: Received, reviewed and accepted class report from RINA stating the deficiency was rectified. Main engine governor has been adjusted and main engine has been checked and found satisfactory.

Resolution: Received, reviewed and accepted class report from RINA stating the deficiency was rectified. New gaskets and connecting bolts were installed on fuel oil pumps for #1 and #2 service generators. Generators tested satisfactory without leakage.
