Every liferaft shall be stowed with its painter permanently attached to the ship. PSCO observed that the painter to the forward liferaft was not attached. 74 SOLAS (14) III/13.4.1 10C

All steering gear components and the rudder stock shall be of sound and reliable construction. PSCO noticed fixed hydraulic fluid storage tank for the steering gear was leaking, creating a puddle on the deck. 74SOLAS(04) II-1/20.2.1 17A/C

A portable foam applicator unit shall consist of an air foam nozzle of an inductor type capable of being connected to the fire main by fire hose. The nozzle shall be capable of producing effective foam suitable for extinguishing an oil fire. PSCO noted kinked discharge hose on the portable foam applicator unit by the engine room escape tunnel and kinked, as well as, cracked. Discharge hose on foam applicator by the purifier room which would prevent the effective production of foam in the event of a fire. 74 SOLAS(01)II-2/6.4 17A/C

The steering gear compartments shall include handrails and gratings or toehr non slip surfaces to ensure suitable working conditions in the event of a hydraulic leakage. PSCO noted broken, raised gratinfs in multiple locations in the steering gear room, creating unsuitable working conditions. 74SOLAS(04)II-1/29.13.2 16A/C

The machinery boiler and other pressure vessels shall be so installed and protected as to reduce to a minimum any danger to the person onboard, due regard being paid to moving parts, hot surfaces, and other hazards. PSCO noted missing exhaust pipe lagging on the #1 and #3 ship service generators (SSDG). Additionally PSCO noted an exhaust leak on the #2 SSDG. 74 SOLAS (04)II – 1/26 17A/C

After survey of the ship under regulations 7, 8, 9, or 10 has been completed, no change shall made in the structural arrangements, machinery equipment and other items covered by the survey without the sanction of the administration. PSCO noted the fuel oil drain pipe to the (Low Sulfur Marine Grade Oil) LSMGO tank was found disconnected and laying on the deck. 74SOLAS(14)I/11(b) 17A/C

The purpose of this regulation is to maintain and monitor the effectivemess of the fire safety measures the ship is provided with. For this purpose, the following functional requirements shall be met. Fire protection systems and firefighting systems and appliances shall be maintained ready for use. During spot check of the emergency generator, the PSCO discovered the switchboard in the manual selection mode. Although posted instructions in the space stated is should have been left in the automatic position prior to securing the space. Additionally, during testing of the quick closing arangements, it was noted the generators fuel oil valvecould not secure the generator. furthermore, the piping bracket which holds the wire rope that operates the valve (in the event of fire) broke away from the bulkhead when the crewmember tested. it. Two attempts by crew to repair valve without success. PSCO believes that these arrangements present a hazardous condition onboard in the event of fire or emergency. 74SOLAS(14)II-2/14.1.1 30A/C

The company should establish procedureds to ensure that the ship in maintained in accordance with the provisions of the relevant rules and regulations established by the company. Due to the objective evidence in the deficiencies listed herein, the vessel is not in substantial compliance with the relevant conventions. The COTP questions the adequacy and/or implementation of the vessels SMS under the ISM codeAn external audit is recommended to be conducted within 30 days by the Flag or RO to determine whether the ship is operating in accordance with the ISM Code. 0715-Fire fighting appliances(Detection)- The condition of the ship and its equipment shall be mantained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to ship or persons on board. PSCO observed the fire control panel displayed a fault which rendered the general alarm inoperable. SOLAS 74 (2001) I/11(a) 17(a)

0620-Inflatable liferafts- Each liferaft or group of liferafts shall be stowed with a float free arrangement complying with requirements of paragraph 4.1.6 of the code so that each floats free and if, inflatable, inflates automatically when the ship sinks. PSCO observed the liferaft inflation line (painter) was incorrectly attached directly to the liferaft cradle. SOLAS 74 (2001) III/13.4.2 10(c)

0635-Launch arrangements for rescue boat- Launching and embarkation appliances complying with the requirements of section 6.1 of the code shall be provided for all survival craft. Where davit arms are recovered by power safety devices shall be fitted wich will automatically cut off power before the davit arms reach the stops. PSCO observed limit switch on winch utilized for rescue boat release hoisting inoperable. SOLAS 74 (2001) III/16.1 10(C)

0899 - Each transfer pipe system used for the transfer of oil or liquid hazardous material to or from a vessel with the capacity of 250 barrels or more must not leak under static pressure at least 1.5 times the maximum allowable working pressure annually. Chief engineer stated that last hydrostatic test of bunker lines was conducted using air as a medium. 33CFR156.170

1730 - Oil filtering equipment referred to in paragraph 2 of this regulation shall comply with paragraph 6 of this regulation. 1420 - The bilge around the main engine is covered in excessive amounts of oil which poses a serious fire and safety hazard to the vessel and crew. In particular, large amounts of insulation and rags were found in the bilge to collect oily waste. 74 SOLAS (14) XI-1/4.1 17abc

1499 - The seawater cooling delivery line from the sea chest to the fresh water heat exchanger is severely corroded and has multiple temporary repairs in place. Furthermore, the heat exchangers for the fresh water cooling have been altered to allow cross connecting of sea water inlet and discharge resulting from the corrosion of the sea water pipe. 74 SOLAS (04) II-1/26.3.6 17 abc

1420 - The fuel delivery valve on the #3 HFO Storage Tank is leaking excessively; the crew has stuffed rags around the valve which poses a fire hazard to the vessel. 74 SOLAS (01) II-2/15.2 17abc

0999 - Modified ball valve fitting to the Dirty Oil Tank hatch cover would not prevent the ingress of water into the engine room in the event of overfilling or flooding from the tank. 74 SOLAS (14) I/11(a) 17abc

0950 - Cables and wiring shall be installed and supported in such a manner as to avoid chafing or other damage. Electrical wiring for lighting in engine room in way of the Marine Sanitation Device is secured with manila line and plastic ties. 74 SOLAS (04) II-1/45.5.5 17 abc

1499 - The fixed storage tank is leaking from the bottom of the sight glass. Crew has stuffed rags into the sight glass to control leaking. 74 SOLAS (04) II-1/29.12.3 17abc

1420 - Signs of cleaning and work continue in void above #2 HFO Storage Tank. Crew claims ABS approved repairs to #2 HFO Storage Tank after damage in June 2015. Class to verify if void space contains fuel oil leaks from the fuel tank. 74 SOLAS (14) XI-1/4.1 17abc

0720 - The 20 liter portable foam extinguishing system on the lower deck of the engine room was not ready for use. The foam agent had leaked out of the bottles and filled the containment box. Crew cleaned the box and replaced foam agent with spares on board.

0663 - The immersion suit storage box in the engine control room was covered in old computer parts and was not available for immediate use. Crew cleaned the area and removed equipment that was in the way of life saving equipment. 74 SOLAS (14) III/20.2 10c

Vessel reported an inoperative gyro compass.

HAD PLASTIC WRAPPED AROUND STERN LIGHT ELECTRICAL HOUSING AS WELL AS VENTILATION CONTROL BOXES.

REPLACE DAMPENER SCREENS ON VENTILATION DUCTS. REPAIR INSULATION HOUSING ON EXHAUST.

The side shell was inset and frame 26 is tripped. HFO tank was empty at the time of the allision.

Deck in Reefer room is soft and rotted due to condensation dripping onto the deck. Ship is to make temporary repairs and make permanent repairs at dry dock.

Vessel is using improper procedure for drawing water from purifier sludge tank via unapproved valve. The draw line valve is to be removed/ blanked off and a tracking seal is to be installed and logged according to ships policy. Company to develop procedure for removing excess water from purifier sludge tank by installing fixed piping.

Oily Water Seperator alarm for oily waste of greater than 15ppm does not function as per system design. However, the automatic stoppage device does function properly.
