The company should ensure that the policy is implemented and maintained at all levels of the organization, both ship-based and shore-based. Based on unreported alterations to the main engine of the vessel and/or unreported variations from the approved manufacturers specifications, the Safety Management System with regards to engineering maintenance and implementation has failed. Recommend audit.
