Fire fighting annual servicing reports must show the accurate weight of each CO2 fixed cylinder. The weights for each cylinder (04) were inaccurate on the net weight. Provide a servicing report that reflects the correct weights.

Vessel master is required to have a FCC Marine Radio Operator Permit. master did not have a valid permit. Provide a valid FCC Marine Radio Operator Permit.

Portable fire extinguishers are required to be inspected monthly. Vessel's crew were unable to provide proof that the extinguishers were inspected monthly. Provide proof that the portable fire extinguishers are inspected monthly, IAW NFPA 10.

A vessel that is exposed to salt water not more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural examination at least once every five years. Vessel was in drydock at Metal Shark Boats in Bayou La Batre, AL but departed before outstanding items were verified by marine inspectors. These outstanding items were: 1. Stern void port side has bag on bilge suction 2. Void 5 from the stern has a RoxBlox that is not watertight, also has a bag on bilge suction. 3. Void 16 found frame and two tripping brackets wasted. 4. Stitch welding of longitudinal frames in engine room 5. 1st frame forward of aft bulkhead inboard in the port machinery space not attached. These items must be verified before DD/ISE credit can be given and deficiency cleared.

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel experienced a momentary loss of propulsion. Company is required to submit a report of findings and test of propulsion equipment, to the satisfaction on the Marine Inspector, prior to having any restrictions lifted. Written authorization of 835 clearance from local OCMI required prior to resuming operation.

Electrical shore tie aft connection bracket welds found broken.

Marker light on port side life ring found broken.

Life buoy water light battery worked but expired, replace with correct battery.

Water light for life bouy needs corrosion clasp for attechment to life ring.

Add 01 to number to stowage of adult life jackets. Needs to read 86 for proper markings.

Both fire fighting cylinders will be hydrostatic tested and filled with the odorizing unit to produce the scent of wintergreen.

Not all engine gauges were properly working at the operating station for engines #3 and #4. All gauges shall be made to operate properly. IAW 46 CFR 182.410.

The panel box in the generator room has open spaces (where circut breakers would normally be placed). All open spaces in the panel box shall be covered and the labeling for the panel box spaces shall be updated. IAW 46 CFR 183.220

The vessel's generator had some exposed / dead-end wiring. All such wiring shall be properly capped / sealed. IAW 46 CFR 183.220

The vessel's bilge manifold system and piping were not properly marked / labeled to indicate spaces served. All bilge manifold and piping shall be properly marked to indicate spaces served. IAW 46 CFR 176.830

The vessel did not have portable lights (flashlights) onboard. Vessel shall provide at least two (recommend three) portable lights. IAW 46 CFR 183.430
