Each window, port hole, and its means of attachment to the hull or deck house, must be capable of withstanding the maximum load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Inboard port fwd window in wheelhouse is cracked on bottom. Replace window prior to next credit dry dock.

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection. Hydraulic bow ram gauge in #2 STBD void is broken. Repair or replace prior to credit next dry dock.

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] in #2 STBD void is buckled. submit repair proposal to our office prior to next credit dry dock.

§ 122.520 Abandon ship and man overboard drills and training. Ladders must be designed with sufficient strength to sustain load of 4.8kPa (100 pounds per square foot) with a safety factor of 4, based on ultimate strength of the material. Starboard Engine Room aft ladder is missing a bolt (top right) to secure the ladder in place. (46 CFR 116.438(b) (16c))

Battery installation. Enclosure box for battery in the portside engine room has a cracked cover that must be fixed or repaired. (46 CFR 120.354 (16c))

Steering system must be maintained to the original approval, Gauge is full of oil/hydraulic fluids & does not operate as designed, must replace to like new condition prior to dry dock credit.

Exhaust bracket located in the portside jet room has significant deterioration. Bracket must be replace/renewed during next dry dock prior to receiving drydock credit. (46 CFR 119.430 (705c))

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Both engine rooms have excessive oil accumulation. Engine rooms must be clean and dry to determine if there are any leaks and to prevent fire or a hazardous environment. Vessel must provide plan on engine room clean up. (46 CFR 115.830(b) (705c))

Coolant gauge on #2 eng located in port eng room is inoperable.

Ring buoy waterlight battery is expired.

Ground detection must be provided for an alternating current system. 1.5 – 2 Ohms Ground detected on electrical panel. Find and fix ground causing circuit

Vessel's DPA must ensure the following crew members get additional safety training prior to crewing on the Seastreak Highlands: Chris Gjersten, [name withheld], [name withheld] Jr, Ray Sanborn & [name withheld].

Vessel must find the source of the 4 amp ground, correct and report findings to the Coast Guard.

Vessel must locate and repair all sources of oil leaking from the engine into the bilge.

Vessel must repair bucket ram seals to prevent increased leakage into the laz compartment. Vessel must clean and maintain bilges at all times.

Vessel must prove proper operation of the starboard hull bilge system. Possible cause is a hole in the piping system.

Vessel experienced a crank case over pressurization and subsequent loss of oil pressure in the #3 engine. Verify repairs with technician report and conduct sea trials in the presence of a marine inspector.

STBD E/R, aft Engine turbo has an exhaust leak, test ran engine. Prior to carrying passengers.

STBD E/R, aft engine has a raw water (sea water) leak at the jacket water cooler. Repair leak, test run engine. Prior to carrying passengers.

STBD Fore Peak bilge high level alarm not working. Prove proper operation. Cleared prior to departure.

Update fire control plan

Main deck aft life jacket locker holds child and adult life jackets, jackets are not easily identified between adult and child life jackets. Separate child life jackets in locker and make obvious between child and adult.

AF01- Vessel had a fire in the stbd jet room from a ruptured hydraulic hose leaking hydraulic fluid onto the exhaust. Vessel must repair/replace hydraulic line and clean the stbd jet room to the satisfaction of the attending USCG MI prior to carrying passengers.

Stbd SSDG windings are saturated in L/O, repair or replace.

Stbd aft ventilation dampener is not working properly, repair or replace.

Vessel must weld all cracked hand rails on port & starboard bow as discussed to complete temporary repair. Permanent repairs must be complete during next dry dock.

Supply and exhaust vents for the engineroom on both port & starboard sides are covered with soot. Clean out vents and ventilation motors.

Provide proper testing equipment for both smoke and heat detectors in accordance with the manufacturer’s instructions.

Provide a copy of maintenance requirements for smoke and heat detectors from the manufacture.

Demonstrating proper operation of dewatering equipment took a great length of time due to the bilge suction foot valves, but does operate. Replace all foot valves on all bilge suction lines and demonstrate proper operation.

Submit operations manual to local OCMI for approval for all high speed ferries operating at speeds of 30 knts or more loaded.

All high speed ferries operating at speeds of 30 knts or more loaded are restricted to having no less than one licensed Master and one licensed mate on the bridge at all times until operations manual is approved by local OCMI.

Repair Anchor Light.

Provide documentation of crew participation in drug and alcohol program.

Repair #1 remote gauges on starboard bridge wing. Rudder indicator (bucket indicator) and Nozzel indicator inoperable.

Excessive oil in the bilges of both engine rooms, port and starboard. Bilges to be pumped free of oil.

Wire(s) run in overhead above #3 main hanging. Properly hang wires using proper strapping.

Properly install 24 inch fans in both engine rooms, or remove.

Fire/GQ alarm in starboard engine room is not working, demonstrate proper operation.

Natural vent duct in both lazarettes not connected, properly attach.

Bolt missing for bottom of escape ladder out of aft starboard engine room, properly attach.

Sea water cooling for main engine port side has a hole in the overboard. Pipe also has crack. Properly repair line. Temporary repair to be completed prior to sailing, permanent repairs by 05 MAR 09.

Replace or fix the guard that protects the jets. Currently, the jet guard shows significant signs of cracks and separation from the hull.

Doubler plates for the exhaust pipes need to be replaced.

Fix a pin hole on STBD catamaran hull by #4 compartment.

Replace #1 fire main sw suction valve (2"ball valve)

33U.S.C.1321(j)(5)(G)-The vessel owner/operator has not provided certification ensuring that response resources are capable of responding to the maximum extent practicable, to a worst case discharge. 33U.S.C.1321(j)(5)(D)(iii)-The NTVRP correspondence identifies an unclassified oil spill removal organization (OSRO) for the New York COTP zone, and does not identify OSROs in the plan.

Vessel main engine turbo supply and drain lines as well as step time comtrol hoses are installed without required fire sleeve. Vessel rep has identified hose as AEROQUIP 2807 series hose. 2807 Hose is approved for use in fuel and oil service on commercial vessels only if the fire sleeve listed in SAE J1942-1 is installed IAW mfg instructions. Provide proper installations.

Provide current training log book IAW 46CFR122.420

Provide copy of liferaft servicing certificate.

Provide lifesaving/lifejacket donning instructions on stowage boxes on bow and port third deck.

Label life ring buoys.

Replace lifering buoy line with Poly line, 65'.

All main engines: Restore exhaust system insulation to original condition. 46CFR58.10(d)

ECM/STC - L/O hoses, replace to meet 46 CFR 56.60-25 on all main engines.

STBD/FWD Main engine, L/O line from block to ECM loose and leaking. Correct.

Both stbd main engine racor filter units leaking IWO pipe elbow. Repair.

Port/aft main L/O hose to bottom of ECM/STC worn through IWO deck plate. Replace.

Both generators; insulate exhaust elbow from turbo. 46CFR58.10(d) via 46CFR58.10-10

Both generators; turbo L/O supply hose must meet 46CFR56.60-25. Correct.

Both generator F/O rtn hose must meet 46CFR56.60-25. Correct.

Stbd generator high pressure F/O pipes to #1 and #6 injectors are in contact and worn by vibration. Replace.

F/O pump on stbd generator leaking. Repair.

Excessive accumulation of oil in bilges. Clean. 46CFR115.830

Port aft engine invloved in a turbocharger failure/engineroom fire. Make proper, permanent repairs IWO the turbocharger on the #2 MDE (port - aft).

Port CO2 bottles discharged during engineroom fire. Issued requirement to re-charge & service. Vessel allowed to transit to maint facility at Pier 81 to effect repairs. Starboard fire pump/hoses must be energized during transit.

Prior to operating vessel with the #3 main engine with passengers, the operator must provide manufactures service report of repairs and to demonstrate proper operation o the #3 main in presence of the USCG Inspector. 10/14/04: Extended requirement to 10/29/2004 due to unavailability of tech rep.
