Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured. Electrical dead-end cable was found on the no.1 diesel generator and is an electrical hazard. 2004 SOLAS(74) II-1/40.1.3

Main and emergency switchboards shall be so arranged as to give easy access as may be needed to apparatus and equipment batteries and chair were stored behind the emergency switchboard and arranged as to not give easy access to apparatus and equipment. 2004 SOLAS(74) II-1/45.2

The machinery shall be so installed and protected as to reduce to a minimum any danger to persons onboard, due regard being paid to moving parts. There was no guard on rotating equipment on the no.2 boiler water circulating pump and poses as a danger to persons onboard. 2004 SOLAS(74) II-1/26.1

001-13102-The machinery shall be of a design and construction adequate for the service for which they are intended. The primary and auxiliary filter banks for main fuel supply for Ship service generators 02, 03 and 04 were blocked limiting fuel supply causing the engine to shut down; resulting in loss of ships electrical power required for ships operation and propulsion. - 74 Solas (2004 Cons.) The Safety Management System (SMS) should ensure compliance with mandatory rules and regulations; and that applicable codes, guidelines and standards recommended by the Organization, Administrations, classification societies and maritime industry organizations are taken into account. The combination ofdeficiencies below indicates a serious failure, or lack of effectiveness, of the implementation of the ISM Code. A safety management audit shall be carried out by the Administration or the RO before the ship may be released from detention. 74 SOLAS (2020 Cons) IX/3.1 ISM CODE 1.2.3, 30a/c.

The condition of the ship shall be maintained. PSCO observed active leaks on the fuel oil purifiers and main engine fuel injectors (cylinders #2 & #8), resulting in excessive oil in the bilges throughout the engine room posing a pollution/fire risk. On March 29, 2023, the ship received a PSC exam in Houston, TX and received a similar deficiency citing excessive leakage in the engineroom. CG Sector Houston required an internal audit be completed within 90 days with "the focus on engine room maintenance and cleanliness." The required internal audit was carried out on 07Jun2023. A review of the resulting audit report by the PSCO did not reveal any particular focus on engineroom cleanliness/maintenance. Further, the excessive oil accumulation in the bilges and fuel oil purifier room constitute objective evidence that any corrective actions which may have been implemented were ineffective. 74 SOLAS (2020 Cons) IX/3.1 ISM CODE 9.2, 30a/c.

Steam pipe systems and fittings shall withstand the max working stress to which subjected. PSCO observed that lagging had been removed from fuel oil heating steam piping in the port tunnel, exposing an active steam leak and hot surfaces. Similarly, a steam leak was noted on the heat exchanger for the lube oil purifier. These conditions poses a burn risk to the crew. 74 SOLAS(2020 Cons) II-1/33.1, 30a/c.

Machinery shall be adequate for intended purpose. Excessive water leakage was noted from two scrubber seawater pumps (P01B and P01C). Multiple soft patch and unapproved repairs were also noted on the seawater scrubber piping. Finally, the sea water piping at the scrubber, located in the top of the engineroom, was leaking and raining down onto the main engine (cylinders 9-12). The excessive crystalized salt build-up on the engine head indicates long term leakage. 74 SOLAS (2020 Cons)II-1/26.1, 30a/c.

PRECAUTIONS SHALL BE TAKEN TO PREVENT ANY OIL THAT MAY ESCAPE UNDER PRESSURE FROM ANY PUMP, FILTER, OR HEATER FFROM COMING INTO CONTACT WITH HEATED SURFACES. PSCO OBSERVED A 5 GALLON BUCKET COMPLETELY FILLED WITH OIL UNDERNEATH THE #1 FUEL OIL TRANSFEWR PUMP, WHICH WAS ACTIVELY LEAKING AT APROX 3 TABLESPOONS PER MINUTE. ADDITIONALLY, PSCO OBSERVED NUMNEROUS PLASTIC CONTAINERS FILLED WITH OIL IN THE PURIFIER ROOM. OIL AND OILY RAGS WERE DISCOVERED THROUGHOUT THE ENGINE ROOM. 74SOLAS (01) II-2/15.4

THE COMPANY SHOULD ESTABLISH PROCEDURES TO ENSURE THAT THE SHIP IS MAINTAINED IN CONFORMITY WITH THE PROVISIONS OF THE RELEVANT RULES AND REGULATIONS AND WITH ANY ADDITIONAL REQUIREMENTS WHICH MAY BE ESTABLISHED BY THE COMPANY. OBJECTIVE EVIDENCE DISCOVERED AS DELINEATED IN DEFICIENCY 01, THE VESSEL FAILED TO FULLY IMPLEMENT THE REQUIREMENTS OF THE SMS. AN INTERNAL AUDIT WITH THE FOCUS ON ENGINE ROOM MAINTENANCE AND CLEANLINESS IS REQUIRED WITHIN 3 MONTHS. 74SOLAS(20) IX/3.1 ISM 10.1

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. The vessel experienced a grounding while transiting the navigable waters of the United States which adversely impacted its fitness and capabilty to safely manuever the Houston Ship Channel. Submit for review a Classification Society Report to [email withheld] attesting to the cause of the equipment failure, that authorized that temporary or permanent repairs have been completed, and that the vessel's full and normal manuverability has been restored.

THE RING, LINES AND LETTERS SHALL BE PAINTED IN WHITE OR YELLOW ON A DARK GROUND OR IN BLACK ON A LIGHT GROUND. THEY SHALL ALSO BE PERMANENTLY MARKED ON THE SIDES OF THE SHIPTO THE SATISFACTION OF THE ADMINISTRATION. THE MARKS SHALL BE PLANLY VISIBLE AND, IF NECESSARY, SPECIAL ARRANGEMENTS SHALL BE MADE FOR THIS PURPOSE. THE LOADLINE MARKINGS ON THE SHIP'S PORT SIDE WERE NOT PAINTED WHITE OR YELLOW ON A DARK BACKROUND, AND NEED TO BE PAINTED IN ORDER TO BE PLAINLY VISIBLE. ICLL 8 40AC

33CFR159.95(a)(1) Each device (MSD) must be free of design defects that would allow toxic substances to escape to the interior of the vessel. The top of the MSD was observed holed.

The vessel has an unexplained electrical malfunction (possibly caused by a short) that sounded the main engine governor alarm. The vessel is in "deadship" condition, though, the bow thruster and rudder are operable. Repair to the satisfaction of class prior to departure. SOLAS 2004 Consolidated/ A-1/ Reg 3-1.
