Ea. fire hose must be lined commercial fire hose that conforms to UL 19 or ... being equivalent in performance. The firehose is torn and leaks at the fitting to the fire hydrant. Replace with suitable fire hose. CITE: Old-T 46CFR§181.15-10

Fire pump must be capable of providing adequate pressure. The pressure from the fire pump was insufficient to fight a fire. Prove proper operation of the vessel's fire pump w/discharge pressure sufficient to fight a fire. CITE: Old-T 46CFR§181.10-1

Ring buoys must be marked with Type II retro-reflective material approved in accordance with 46 CFR 164.018. The ring buoys did not have retro-reflective material. Apply retro-reflective material to the ring buoys. CITE: 46 CFR §185.604(i)

For each marine casualty required to be reported ... the vessel shall determine whether there is any evidence of alcohol or drug use. The alcohol test swabs were out of date. Replace with unexpired swabs. CITE: 46 CFR §4.06-15(a)

VHF radio w/DSC must be installed on dedicated, properly protected circuit. Both VHF radios are on the same circuit. Properly install VHF-DSC radio on dedicated circuit.

A First Aid Kit is to be provided onboard. Items in the 1st Aid Kit were out of date. Replace expired items. CITE: 46 CFR §184.710

The discharge ends of fuel tank vent pipes must be fitted with removable flame screens or flame arresters. The flame screens must consist of a single screen of corrosion resistant wire of at least 30 × 30 mesh. CITE: Old T 46 CFR §182.15-35(d)

Dischg of pre-engineered Fireboy system must shut down power ventilation in ER. Power ventilation blowers are not connected and do not shut down when Fireboy is activated. Properly wire power ventilation to shutdown upon Fireboy activation. (Good marine practice: It was noted that the ER ventilation closures were not marked and that the crew was unfamiliar with how to close the vents. It is recommended to positively mark vents, familiarize crew in process for their closure, and incorporate the procedures into vsl's SMS manual.) CITE: 46 CFR §181.420(a)(3)

Blanks must be installed in electrical panels to prevent inadvertent contact with electric circuits. Blanks missing from panel behind helm seat. Properly install blanks. CITE: Old-T 46 CFR §183.10-10

Add chaffe gear to flexible fuel hoses in tank space compartment. 46 CFR 176.800

Battery chargers must be fitted with ammeter connected in the charging circuit. Dolphin Pro battery charger installed in the engine room was not observed having an ammeter. 46 CFR 183.350(f).

Wire nuts on electrical splices need to be in an enclosure and the insulated cap of the connector must be secured to prevent loosening from vibration. Splices leading from the fire boy were not in an enclosure or secured. Place splices in enclosure or make permanent type connections. 46 CFR 183.340(g).

Owners/Operators must mitigate hazards from flammable liquids. Fuel piping on starboard side of fuel tank compartment was weeping fuel and wrapped in diapers to prevent leakage to the bilge. Repair piping and/or connections to stop leakage.46 CFR 176.830.

A pre-engineered fixed gas fire extinguishing system must have a means to reset devices used to automatically shut down ventillation systems and engines at the operating station. The Captain was unable to restart the starboard engine after exercising the override.

Individual power pumps used for separate spaces are to be controlled from a central control point and must have a light or other visual means at the control point to indicate operation. Fuse holder and fuse for fwd compartment bilge pump are missing at the control station rendering the bilge pump inoperable.

Each vessel that operates on the high seas must have on board a FCC type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. EPIRB was not installed in an automatic float free arrangement.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Paper charts are out of date and no longer valid. If electronic charts are utilized as primary source of navigation; documentation must be submitted to verify they are of the approved type.

Each battery must be located as high above the bilge as practicable. One set of batteries are stored in the forward bilge of the engine room space.

Repairs or alterations to the fire detecting or fire extinguishing equipment must not be made without the approval of the cognizant [name withheld] extinguishing system was replaced with new clean agent pre-engineered system without cognizant OCMI acknowledgment. Provide evidence that the system is proper size and is installed in accordance with the manufacturer's instructions.

Life jackets must be stored in convenient places and if kept in a stowage container must be readily available. Adult life jackets are kept in an enclosed space which can only be accessed by reaching behind an emergency escape ladder, which is not a convenient or readily available arrangement.

The Certificate of Inspection and Stability Letter must be framed under glass or other transparent material and posted in a conspicuous place. Neither document was posted at time of inspection.

At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each lifeline must be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. The vessel is certificated to carry one ring lifebuoy, however choses to have two. Remove the lifeline from one of the two and install an appropriate UV resistant lifeline.

Electrical equipment on a vessel must be installed and maintained to minimize personnel contact with energized parts. The panel located behind the operating station had openings and a broken breaker. Place blanks on openings and replace broken breaker.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure. There were cables spliced throughout the vessel, particularly in the engine space that were not within an enclosure. For cables that were spliced, ensure that the new cable is a stranded copper conductor with sufficient current carrying capacity for the circuit in which they are used. Prove that the cable spliced into the bucket control is appropriately rated and of marine grade. In addition, all cable and wire must be installed to avoid chafing and other damage. There were multiple cables throughout engine space that are significantly chafed. Replace damaged cable and ensure no cable or wire is running through the bilge.

Ground detection for each alternating-current system that has a grounded neutral must have an ammeter that indicates the current in the ground connection. There is currently no ground detection for the system. Install appropriate ammeter.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 2 inches high: "Emergency Exit, Keep Clear". The escape hatch in the engine room was not properly marked.

Ensure at least one VHF radio has Digital selective Calling (DSC) capabilities with the vessel's MMSI number and GPS position programed in accordance with 47 CFR 80.1085(a)(2). The vessel will be restricted to carrying passengers if this requirement is not corrected no later than March 19, 2019.

Provide a current and valid Communications Act Safety Radio-telephony Certificate in accordance with 47 CFR 80.401. The vessel will be restricted to carrying passengers if this requirement is not corrected no later than March 19, 2019.

Replace plastic through-hull fitting with metallic in aux space prior to completion of drydock.

Determine vessel's scantlings for below the waterline.

Properly wire and connect the starboard engine to the fixed fire extinguishing system.
