The owner, charterer, master or managing operator shall instruct each crew member, upon first being employed and prior to getting underway for the first time on a particular vessel and at least once every three months, as to the duties that the crew member is expected to perform in an emergency including, but not limited to, the emergency instructions listed on the emergency instruction placard required by §?185.510 of this part and, when applicable, the duties listed in the station bill required by §?185.514 of this part. As noted in the official log, 04 crew members had been assigned/sailed between the dates of 01JAN26 and 11MAY26. There were no records of drills or crew training made available. A Letter of Warning has been drafted and referred to enforcement. Drills were conducted on the spot to clear this CG-835v. 46CFR 185.420

Vessel must meet 46CFR182.420(a) for engine cooling. Vessel had a raw water leak on the engine cooling line on the starboard engine in way of the double hose clamp on the aft quarter of the engine. Make permanent repairs to leaking pipe.

) Irrespective of build date, a vessel to which this subchapter applies must meet 46 CFR 181.405, 181.450, 181.500, 185.364, and 185.420(b), if it is not a ferry, and if it— (1) Has overnight accommodations for passengers; or (2) Is operating on a Coastwise or Oceans route. Vessel must have an interconnected fire detection and alarm system in accordance with 46CFR181.50. Vessel is required to install an interconnected smoke detection system prior to sailing on a Coastwise or Oceans route with more than 6 passengers.

Means of escape shall be marked "EMERGENCY EXIT KEEP CLEAR" in 2" lettering. Emergency escape hatches on deck and wheel house were not labeled and wheel house escape hatch on had equipment stored on top of it. Label all emergency escape hatches.

Vessel is required 6 orange smoke flares. Vessel had 5 orange smoke flares on board. Obtain one more orange smoke flare.

Lighting fixtures must be installed per 46CFR183.200. Lighting fixture in the lazarette is broken and hanging by the electrical wire. Replace lighting fixture.

Provide approved hydraulic system plans for the installation of the bow thruster. Bow thruster must remain tagged out until plans are approved by the USCG Marine Safety Center.

Nonconducting mats or grating must be provided on the deck in front of each switchboard ... The TIOGA is currently missing the matting in front of the switchboard. Provide suitable placed non-conductive matting to the satisfaction of the attending Marine Inspector. 46 CFR 183.330

A vessel must carry either a first-aid kit approved under approval series 160.041 or a kit with equivalent contents and ... The TIOGA first-aid kits on board are expired, provide or renew. 46 CFR 184.710

A vessel must comply with the applicable design, equipment, personnel, procedures, and record requirements of 33 CFR parts 151, 155, and 156 ... The vessel containment drain plug/closure, for the fuel/oil venting, was incapable of securing due to thread conditions. Provide proper closure device for the containment to function as intended. 46 CFR 184.702

Complete COI inspection. Vessels COI has expired.

Vessel's starboard main engine failed while vessel was underway. Three ceramic cam followers had broken into several pieces on cylinders 1, 4, and 5. Vessel will need to have a MTU tech diagnose and provide a repair proposal. Vessel shall not get underway with passengers until repairs have been completed to the satisfaction of a CG Marine Inspector.

Vessel must provide up to date tide tables and current tables.

Vessel must provide up to date charts.

Bilge high level alarm located in the lazarette not operational. Demonstrate proper operation IAW 46 CFR 182.530(a)(1).

Ensure the testing of newly installed machinery is in accordance with CG-CVC Policy Letter's, 17-08 and 17-08 (Enclosure 1).

Vessel does not have up to date charts. Provide up to date charts.

Vessel does not have tides and current publication. Provide publication.

Vessel's aft lazarette high water alarm is not working.

Install new cutlass bearings
