14104 - Pollution Prevention - The PSCO belives that there are clear grounds for believing that the master or crew are not familiar with essential shipboard procedures relating to the prevention of pollution by oil MPEC 107(49) states "Ship staff training should include familiarization in the operation and maintenance of the equipment." A new oil content meter was installed 17MAY2019. The engineering department was unaware how to retrieve information from the oil content meter.

PSCO observed that the IOPP certificate reflected that the oil content meter is apporved in accordance with MPEC 60(33) however vessel is fitted with an oil content meter that is apporved in accordance with MPEC 107(49). Provide clarification from classification society reflecting correct information.

PSCO observed mulitple fuel oil leaks on the #1 and #2 generators on multiple cyclinders and a fuel oil leak on the #4 cyliner of the main diesel engine.

The crew was unable to provide any documentation demonstrating that the crew made the proper notifications per their SMS to their company, class, and flag to inform them of the temporary repairs, or that the temporary repairs were approved by class. When entering the pump room, PSCO observed the alarm had activated despite the space being recently certified safe for workers by a Marine Chemist. The PSCO questioned the vessel's Pumpman and 3rd Officer about the alarm, who explained that a ballast line had gasket had leaked in the space, flooding the lower gas detection sensors.
