EACH SECTION OF SPRINKLERS SHALL BE CAPABLE OF BEING ISOLATED BY ONE STOP VALVE ONLY. THE STOP VALVE IN EACH SECTION SHALL BE READILY ACCESSIBLE AND ITS LOCATION SHALL BE CLEARLY AND PERMANENTLY INDICATED. PSCO OBSERVED VESSEL'S FIRE DOOR 15.3.01 LOCKED AND UNABLE TO BE OPENED BY CREW, WHICH BLOCKED ACCESS TO SECTION VALVES S86 & S87. 74 SOLAS (01) II-2/12.2.2

DOORS AND DOOR FRAMES IN "B" CLASS DIVISIONS AND MEANS OF SECURING THEM SHALL PROVIDE A METHOD OF CLOSURE WHICH SHALL HAVE RESISTANCE TO FIRE EQUIVALENT TO THAT OF THE DIVISIONS. PSCO OBSERVED UNSAT DOOR LATCH ON DECK 7 MVZ 6 STBD SIDE, NOT ALLOWING THE DOOR TO MEET A "B" CLASS DIVISION.

ANY REQUIRED AUTOMATIC SPRINKLER SHALL BE CAPABLE OF IMMEDIATE OPERATION AT ALL TIMES AND NO ACTION BY THE CREW SHALL BE NECESSARY TO SET IT IN OPERATION. PSCO OBSERVED IRREGULARITY IN AMPULE IN PANTRY DECK 10 MVZ 5.

Doors and door frames in B class divisions and means of securing them shall provide a method of closure which have the resistance to fire equivalent to that of the division. PSCO observed a self-closing B-15 door not closing, and remaining open in Deck 5, art gallery. 74SOLAS(01)II-2/31.1.1

THE DAMPER SHALL BE PROVIDED WITH AN INDICATOR WHICH SHOWS WHETHER THE DAMPER IS OPEN OR CLOSED. PSCO OBSERVED NUMEROUS FIRE DAMPER INDICATOR LIGHTs WERE INOPERABLE AS FOLLOWS: #FS/ES06101/1; #CO/SS06101/1; #FS/ES06101/1; #CO/EG.08.3.01; #CO/EG.08.3.01/3; #CO/EG.08.3.01/5; #FD/EG16610/02; #FS/VE01603/1; #FS/VSO2101/1; #FS/VE02101/1; #FD/RP08302/1; #FD/AP08302/1. 74 SOLAS (01) II-2/16.2.2

ALL FIRE DOORS IN STAIRWAY ENCLOSURED, MAIN VERTICAL ZONE BULKHEADS AND GALLEY BOUNDARIES WHICH ARE NORMALLY KEPT OPEN SHALL BE CAPABLE OF RELEASE FROM A CENTRAL CONTROL STATION AND FROM A POSITION AT THE DOOR. PSCO OBSERVED FIRE SCREEN DOOR IN THE GALLEY DECK 6 INOPERABLE; AND SELF-CLOSING FIRE DOOR # FSD 4.7.15 AFT AT I-95 NOT CLOSING.

A FOOD RATION TOTALING NOT LESS THAN 10,000 KJ FOR EACH PERSON THE LIFEBOAT IS PERMITTED TO ACCOMMODATE; THESE RATIONS SHALL BE KEPT IN AIRTIGHT PACKAGING AND STOWED IN A WATERTIGHT CONTAINER. PSCO OBSERVED LIFEBOAT RATIONS IN LIFEBOATS 5, 7, 10, 12, 22, AND 25 THAT LOST AIRTIGHT INTEGRITY. SOLAS(20) III/34 LSA CODE 4.4.8.12

Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. PSCO observed second in command on lifeboat #24 unable to start the lifeboat. 74 SOLAS III/19.2.1

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces at least one of which shall give access to a stairway forming a vertical escape. PSCO found the sauna escape access door blocked and inoperable. 74SOLAS01/II-2/28.1.2.

It shall be possible for each door to be opened and closed from each side of the bulkhead by one person only. PSCO found fire doors FSD 10.4.05 not latching and secondary door not activating, FSD 1 4.05 not latching and remaining in open position, and FSD6.2.02 at the theater closing too quick. 74SOLAS01/II-2/30.3

The provisions on the prevention of accidents and the protection of health in employment which may be applicable to the work of seafarers, and shall specify measurers for the prevention of accidents which are peculiar to maritime employment. PSCO found a raised ramp located by the rescue boat starboard side posing a tripping hazard. ILO-147/ PWSA 33USC1223(b).

Two means of escape shall be provided from each machinery space. PSCO observed evacuation route fire door (1.5.04) in purifier room was unable to be opened due to pressure and ventilation in the space. 74 SOLAS (01) II-2/28.3.1 Code 50bc

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. PSCO observed water dripping out of light fixture above computer area into containment bucket in the printer room. 74 SOLAS (04 II-1/45.1.3 Code 10c

Fire hoses shall be of non-perishable material approved by the Administration and shall be sufficient in length to project a jet of water to any of the spaces in which they may be required to be used. Each hose shall be provided with a nozzle and the necessary couplings. In interior locations in passenger ships carrying more than 36 passengers fire hoses shall be connected to the hydrants at all times. PSCO observed fire hose not connected to fire main in the MBR room. 74 SOLAS (01) II-2/4.7.1 Code 10c

Doors and door frames in "B" class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions, ecept that ventilation openings may be permitted in the lower portion of such doors. Where such opening is in or under a door the total net area of any such opening or openings shall not exceed 0.05 m2. When such opening is cut in a door it shall be fitted with a grill made of non-combustible material. PSCO observed B-15 doors 15709, by room c306, aft crew stairway deck 15, and hospital space door 4404 with damaged hinges that would not shut and allow for the passage of flame. 74 SOLAS (01) II-2/31.1.1 Code 10c

All ships of 500 gross tonnage and upwards shall have a gyro-compass heading repeater, or other means, to supply heading information visually at the emergency steering position. STBD side steering room gyro-compass was displaying error code. 74 SOLAS (20) V/19.2.5.2 Code 10c

The construction of all doors and door frames in "A" class divisions, with the means of securing them when closed, shall provide resistance to fire as well as to the passage of smoke and flame, as far as practicable, equivalent to that of the bulkheads in which the doors are situated. PSCOs observed FSD 6.1.03 for room 6123 "Daily paint store" being held open with a hold-back device preventing this door from providing resistance to fire as well as smoke. Door was held open by a coat hanger. 74 SOLAS (01) II-2/30.2 Code 10c

All life saving appliances and arrangements shall comply with the applicable requirements of the code thje marking required on liferafts shall be SOLAS "A" pack in block capitals od the Roman alphabet. PSCO's observed 04 life rift containers without SOLAS "A" pack markings. 74 SOLAS (20) III/34 LSA Code 4.1.5.2 Code 50bc

Stairways and ladders shall be arranged to provide ready means of escape to the lifeboat and liferaft embarkation deck from all the passenger and crew spaces and from spaces in the which the crew is normally employed, other than machinery spaces. PSCO found multiple crew cabin cooridoors being obstructed by service carts and personal belongings preventing an immediate access to the emergency exits. Also, on deck four, there were several areas with maintenance equipement and boxes of supplies preventing crew from having access to means of escape. 74SOLAS(2001 CONS.)II-2/28.1 10C

The activation of any detector or manually operated call point shall initiate a visual and audible fire signal at the control panel and indicating units. If the signals have not received attention within 2 min an audible alarm shall be automatically sounded throughout the crew accommodation and service spaces, control stations and machinery spaces of category A. The manually operated call point on deck 7, number 31 105, was tested and received no visual or audible signal at the control panel or at the unit. After failure of the testing unit, crewmember broke the glass on the manually operated call point, still no audible or visual was shown on the control panel or on the unit.

On voyages of less than 96 hours, an NOA must be submitted before departure, but at least 24 hours prior to entry. Vessel failed to properly submit NOA prior to entry to Sector SENE COTP Zone, from previous port of Baltimore, MD

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs in the protection of openings and penetrations in divisions. Two B15 fire doors leading to the "Bell Box" galley space on Deck 11, Zone 6 were found with rags tied around the door latch and handle preventing the door from securely closing. 74 SOLAS 20 II-2/14.2.1

The purpose of this regulation is to provide means of escape so that persons onboard can safely and swiftly escape to the embarkation deck. The following functional requirements shall be met: safe escape routes shall be provided and additional aids for escape shall be provided to ensure adequate design for emergency situations. On deck 11, zone 6 in the "Bell Box" galley space, the chef's office has no installed PA system and a window which was blocked by papers and signage. This installation does not allow for immediate notification in event of emergency when door is shut. 74 SOLAS 20 II-2/13.1.3

Indicating units shall, as a minimum, denote the section in which a detector or manually operated call point has operated...One indicating unit shall be located on the navigation bridge. Manual calls points 3.98 and 1.2 indicated local activation, but no indication was received on the bridge. SOLAS 01 II-2/13

Occurrence of a fault condition shall initiate a visual and audible fault signal at the control panel. Manual call point and smoke alarms in the incinerator room did not alarm at the indicating unit located on the bridge. Smoke detectors 01.1 and 03.125 were not being received on the bridge as activated. SOLAS 01 II-2/13.1.2

Furniture shall not be permitted in the passenger and crew corridors forming escape routes in cabin areas. PSCO's observed a series of trolleys stowed in a crew cabin corridor, by crew stateroom 4307, chairs and boxes located on the forward crew corridors on deck 4, and extra chairs in the corridor located inside the medical space on deck 4. SOLAS 01 II-2/41-2.4.10

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. PSCO observed the #21 lifeboat was not operational when tested during drills due to faulty starter. 17c 74 SOLAS (14) III/20.2

The appropriate time of closure for hinged fire doors shall be no more than 40s and no less than 10s from the beginning of their movement with the ship in the upright position. PSCO observed FSD 11.4.12 closed too slow, FSDs 7.4.09 and 7.3.09 closed too fast, and FSDs 16.7.05 and 14.7.08 did not close. 50ac 74 SOLAS (01) II-2/30.4.2

Main and auxiliary machinery essential for the propulsion, control and safety of the ship shall be provided with effective means for its operation and control. Port propulsion motor suffered a failure of the aft regulator control card. 74 SOLAS(04 CONS) CH.II-1/REG 31.1

9900-Electric Radiators shall be fitted with an element so exposed that clothing, curtains or other similiar materials can not be scorched or set on fire by heat from the element. Heating element id in a fixed position causin scorching on the bulkhead in the Sauna. Solas (01) II-2/18.3 - 16ac

0750- A class divisions are those divisions formed by bulkheads and decks which comply with the following: They shall be insulated with approved Non-combustable material. PSCO observed torn/missing insulation in fan room port and stbd side on deck 16. SOLAS (01) II-2/13.3 - 16ac

Below the bulkhead deck two means of escape at least one of which shall be independent of watertight doors. PSCO observed closed gate device in pressig room preventing means of escape. SOLAS (01) II-2/28.1.1 - 16AC

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons onboard. Port propulsion electrical aft motor coils 171, 172, and 173 were found damaged by the end winding area indicating insulation breakdown. 74Solas Ch1/Reg11(a)

0925: The person in charge of the survival craft shall see that the crew under his command are acquainted with their duties. At least 5 lifeboats were observed during abandon ship drill with either the person in charge or second-in-command not knowing how to start the lifeboat. 74SOLAS(14)III/10.5 17c

0299: Seafarers shall receive safety fam & basic training IAW A-VI/1 of STCW. Designated stairway guides do not have the basic training completed as required by STCW(11)VI/1.2. Full def located in narrative. STCW(11)VI/1.1 40bc
