PSCO was notified via Pilot that the engine on the freefall lifeboat was not operational prior to departure from Grays Harbor, WA. Vessel is allowed a one-time transit from Portland, OR to San Francisco, CA following their flag dispensation for INOP freefall lifeboat engine and submitted voyage plan. 74 SOLAS 20 III/20.2, 17(c)

The ship/shore safety checklist item #7 and DOI item #9 stated that hoses were inspected prior to cargo ops and found suitable. Crew did not follow proper procedures required by 33 CFR 156.120 i,q, w(12) to notify facility and shutdown cargo operations
