Cargo ships with totally enclosed lifeboats shall comply with 4.6 of the Code. All lifeboats shall be properly constructed and shall be capable of maintaining positive stability in the upright position. Starboard lifeboat keel has exposed fiber glass below the waterline.

The safety management system shall be maintained in accordance with the provisions of the International Safety Management Code. The Company should establish procedures to ensure that the ship is maintained... In meeting these requirements, the company should ensure that ... any non-conformity is reported. Crew member was aware of damage on lifboat and did not inform Company of Classification. Based on exam findings, USCG recommends an external audit of the vessels SMS within the next 30 days.

Storage, Settling or daily service tanks having a capacity of 500 Litters and above situated above the double bottom shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned... When PSCO tested quick closing valve on Emergency Generator, the cable outside the space snapped. Crew was able to replace and demonstrate additional tests.
