PSCO identified vessel had not completed company required load test per safety management system. Crew informed PSCO only simulation of load test procedures were being conducted and the emergency generator was not being placed under electrical load. PSCO identified as per company maintenance instructions, that the emergency generator shall be placed under electrical load for 30 minutes.

Any required fixed fire detection and fire alarm system with manual operated call points shall be capable of immediate use at all times. PSCO observed four system faults and one pre-warning that were present on the fire detection panel in the fire control room. Specifically, two smoke detectors near the boiler and one manual call point in the vessel’s fiddley were not operational. Due to panel’s system showing the date 9/14/31 repeatedly, it was unknown to the PSCO how long these faults have been present.

PSCO observed the following: The #2 generator seventh cylinder with active exhaust leak. The #3 generator was observed to have oil leaking from the bottom of the turbocharger. Additionally, an excessive fuel oil leak was observed in the vicinity of all four generators fuel racks.

PSCO observed port side quick release life buoy's light and smoke detached. Additionally, PSCO reviewed vessels LSA weekly interval maintenance, which revealed on 06 JUNE 2026 all LSA equipment was found in good order, counter to what the PSCO identified.

15109- The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. SMS does not adequately provide procedures or checklists for engine room cleanliness and equipment condition checks, resulting in an excessive buildup of oil throughout the engine room that has gone unreported and unattended to, creating a safety hazard and fire hazard throughout. Due to the objective evidence in the above deficiencies, the vessel is not in substantial compliance with relevant conventions. The Captain of the Port questions the adequacy and overall implementation of the vessel's SMS under ISM Code. An internal audit is required within 90 days to determine whether the ship is operating in accordance with ISM Code. Provide a copy of the audit report to USCG when completed.
