PSCO OBSERVED THE MDO SERVICE TANK AND DAILY LUBE OIL TANK'S QUICK CLOSING ARRANGEMENT USE AND NOT RESET FOR READILY AVAILABLE USE. BOTH THE FUEL OIL AND LUBE OIL VALVES WERE IN THE OPEN POSITION.

ADDITIONALLY PSCO OBSERVED MISSING SAFETY CHAINS AND MIDDLE/BOTTOM RUNGS MISSING FROM RAILINGS IN-BETWEEN CARGO HOLDS. FINALLY, PSCO OBSERVED DOGS ON MULTIPLE CARGO HOLDS NATURAL VENTILATION HATCHES SEIZED REQUIRING CREW MEMBERS TO USE A HAMMER TO OPERATE.
