Each Class 1 door on a vessel in ocean service must be designed to with stand a head of water equivalent to the depth from the sill of the door to the margin line but in no case less than 10 feet (3.05 meters). The watertight door located in the engine room stbd aft will not close enough to be substantially tight. In addition the bulkhead in each machinery space of each OSV must be watertight to the bulkhead deck. Above this door are openings (holes) that do not meet watertight integrity standards. Adjust or replace door and make openings above the door watertight prior to going offshore.

Upon arrival, MIs were told the lifebuoys are maintained in a storage locker so that they are not lost overboard. MIs instructed that they must be stowed IAW regulations and safety plan. Provide proper stowage arrangement for lifebuoys IAW safety plan and regulations prior to departure.
