A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Vessel's floating waterlight attached to ring buoy is inoperable. 46 CFR 180.70 (d)(1)

The equipment required for lifefloats and buoyant apparatus must meet the following specifications: The light must be a floating waterlight approved under approval series 161.010 or other standard specified by the Commandant. The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. FWD Life float light had the incorrect battery installed. 46 CFR 180.175(f)(4)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). Vessel does no have functioning GPS on it's VHF radio, rendering the DSC function inoperable. 47 CFR 80.905(a)(1)

A lighting fixture may not be used as a connection box for a circuit other than the branch circuit supplying the fixture. Identified dead end wires tied into lighting circuit that was previously used for fish holding tank. Crew removed dead end wires. Deficiency rectified while MI was on board. 46 CFR 183.410 (b)

Each lighting fixture globe, lens, or diffuser must have a guard or be made of high strength material, except in an accommodation space, radio room, galley, or similar space where it is not subject to damage. Light over aft cabin door on deck did not have cover. Crew installed light with cover, deficiency rectified. 46 CFR 183.410 (a)

Each life jacket, immersion suit, and ring life buoy must be marked in clearly legible block capital letters with the vessel's name. Vessel's name was faded on ring life buoy. Vessel stenciled name on buoy, deficiency rectified. 46 CFR 185.604(b)

Each vessel must have a stability letter issued by the Coast Guard before the vessel is placed into service. This letter sets forth conditions of operation. Vessels stability letter states bilges shall be kept pumped to a minimum at all times. While attending the vessels scheduled dry dock exam, the fwd most void was completely pressed up with water.

Vessels stability letter requires sixty 40 lb concrete blocks located in the forward stowage hold bilge. The vessel does not have the appropriate weight in concrete blocks. Vessel shall weigh all 60 blocks with a certified scale in the presence of a Marine Inspector. The difference in weight will need to be corrected prior to vessel carrying passengers.

Each survival craft must be: Stowed in a position that is readily accessible to crew members for launching, or else provided with a remotely operated device that releases the survival craft into launching position or into the water. Lifefloat stored on the top deck, while the access ladder is located in a void space below, making the lifefloat not readily accessible in an emergency. Install appropriate means to ensure quick and safe access to the lifefloat to the satisfaction of the marine inspector. 46 CFR 180.130 (a)(3)

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. The ladder used in Man Overboard recovery does not sufficiently hook to the vessel’s rails, making it prone to dislodging in sea conditions. Implement measures to ensure the recovery ladder is securely fastened during operations. 46 CFR 180.175(a)

Each small battery installation must be located in a well-ventilated space and protected from falling objects. Battery was missing lid or any protective mechanism. Provide battery casing that protects the battery to the satisfaction of the marine inspector. 46 CFR 183.354(b)

Each hatch exposed to the weather must be watertight. Excessive water was in 3rd void space below hatch on port side. Ensure that gasket/weatherstripping is properly aligned and installed around the entire perimeter of the hatch to create a uniform and secure seal. 46 CFR 179.360(a)

Each vessel must be equipped with at least two operable portable battery lights. One of these lights must be located at the operating station and the other at the access to the propulsion machinery space. No functional flashlights were onboard. Obtain at least 2 and store them in the appropriate areas. 46 CFR 183.430

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station:(1) A light to indicate discharge;(2) An audible alarm that sounds upon discharge. There was no audible alarm when testing the Fireboy. Repair or install properly functioning audible alarm. 46 CFR 181.420(b)

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Portside engine guard did not extend down far enough to fully envelope rotating hazard. Engine guard should be extended or modified to fully enclose the rotating equipment. 46 CFR 177.960

The floating waterlight must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. The waterlight line was only 12 ft. Obtain a longer line to meet the 18 ft. requirement. 46 CFR 180.175(f)(4)

A ship, except a ship of less than 26 feet in length, must have a placard of at least 5 by 8 inches, made of durable material fixed in a conspicuous place in each machinery space, or at the bilge and ballast pump control station, stating the following: "Discharge of Oil Prohibited. The Federal Water Pollution Control Act prohibits the discharge of oil or oily waste into or upon the navigable waters of the United States, or the waters of the contiguous zone, or which may affect natural resources belonging to, appertaining to, or under the exclusive management authority of the United States, if such discharge causes a film or discoloration of the surface of the water or causes a sludge or emulsion beneath the surface of the water. Violators are subject to substantial civil penalties and/or criminal sanctions including fines and imprisonment." No Oil Discharge Prohibited placard was found. Securely affix placard in a visible location. 33 CFR 155.450

A submersible electric bilge pump may be used as a power bilge pump only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers provided that:(1) The pump is permanently mounted. Pump in steering void was not secure. Secure the bilge pump to a stable and appropriate mounting structure. 46 CFR 182.520(e)(3)

Each battery must be located as high above the bilge as practicable, secured to protect against shifting with the roll and pitch of the vessel. Battery was in a tray that was not mounted or strapped to prevent shifting. Mount or strap the battery tray securely. 46 CFR 183.350(b)

The name of the vessel must be marked or painted in clearly legible letters on each life float paddle. "Buccaneer" was faded on one of the paddles. Rewrite vessel name for better visibility. 46 CFR 185.604(g)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements:(1) Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. The master did not have a MROP. A MROP must be obtained. 47 CFR 80.159(c)

The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations to the cognizant [name withheld] alterations must be approved by the OCMI before work is started. Visibility out of the entirety of the vessel's port and starboard windows restricted to window slide inserts. Vessel to submit plan to cognizant OCMI for review and approval to repair/replace the entirety of port and starboard windows in the wheel house.

In the application of inspection standards due consideration must be given to the hazards involved in the operation permitted by a vessel's Certificate of Inspection. Thus, the standards may vary in accordance with the vessel's area of operation or any other operational restrictions or limitations. Vessel utilizing electronic charts with designated tablet not securely mounted to vessel. Vessel to have a secured mounting for the designated tablet at the operating station in wheel house to the satisfaction of the USCG Marine Inspector.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of electrical equipment and systems, including the following: Inspection of all cable as far as practicable without undue disturbance of the cable or electrical apparatus. Electrical box on generator missing cover.

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. Vessel did not have on board three fire buckets.

Glass or other glazing material used in windows at the operating station must have a light transmission of not less than 70 percent according to Test 2 of ANSI Z 26.1 (incorporated by reference, see 46 CFR 175.600) and must comply with Test 15 of ANSI Z 26.1 for Class I Optical Deviation. There is no visibility out of the port and starboard windows in the wheel house as the material used obstructs the visibility. Rectify to satisfaction of USCG Marine Inspector prior to movement.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel's charts were not current. Rectify to satisfaction of USCG Marine Inspector prior to movement.

Each portable fire extinguisher on a vessel must be of an approved type. The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b) and other provisions of this section. Vessel did not have the minimum required fire extinguishers on board. Rectify to satisfaction of USCG Marine Inspector prior to movement.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals approved in accordance with § 160.021 in subchapter Q of this chapter, or other standard specified by the Commandant, including, but not limited to, approval series 160.121; and six hand orange smoke distress signals approved in accordance with § 160.037 in subchapter Q of this chapter, or other standard specified by the Commandant. All flares and smoke distress signals were expired. Rectify to satisfaction of USCG Marine Inspector prior to movement.

The following spaces must be equipped with a fixed gas fire extinguishing system, in compliance with § 181.410, or other fixed fire extinguishing system specifically approved by the Commandant, except as otherwise allowed by paragraph (b) of this section: A storeroom containing flammable liquids. Vessels has flamables liquids stored in the forward space. Bring space into compliance or remove flammable liquids.

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. Both life floats were in poor condition and deteriorating with cracks and gouges, and falling apart. Rectify to satisfaction of USCG Marine Inspector.

The number of persons capacity must be marked or painted in clearly legible letters and numbers on each life float and buoyant apparatus. This number must: not be greater than the number of persons the device is approved for as shown on its nameplate. Both life floats missing name plate.

The float-free link required by § 180.130(a)(1) must be:Of proper strength for the size of the life float or buoyant apparatus as indicated on its identification tag. The float-free link was missing identification tag.

Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. Vessel was missing one light for a life float.

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Vessel did not have separated storage for Child size life jackets.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Identification of the items may be in words, or the appropriate symbols in IMO Resolution A.760(18) (incorporated by reference; see 46 CFR 175.600). Letters and numbers must be at least 50 millimeters (2 inches) high. Symbols must be at least 100 mm (4 inches) square. Vessel did not have life jacket storages clearly marked.

Each life jacket must be marked with Type I retroreflective material approved in 46 CFR 164.018 or other standard specified by the Commandant. 04 adult and 02 child life jackets had deteriorated retroreflective material.

A vessel must carry one floating waterlight. The vessel's floating waterlight attached to the ring life buoy was inoperable.

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel's engine room, starboard side, had a wire not connected to any equipment coming from battery.

An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Both port and starboard engines' rotating machinery exposed and not protected.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Vessel had wiring not supported by metal brackets in the accommodation space, engine room, and aft deck.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of machinery, fuel, and piping systems, including the following: Inspection of all machinery essential to the routine operation of the vessel including generators and cooling systems. Prior to inspection, vessel reported inoperable starter on the generator.
