The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or persons on board. Vessel experienced, prior to departing, equipment failure on the two three way valve on the main engine. you are required to submit a classification society report to [email withheld] attesting to the cause of the equipment failure, that authorized temporary or permanent repairs have been completed, and that the vessel's full main engine function has been restored. 74 SOLAS (14) I/11(a) 60ac

Whenever...a defect is discovered which affects the safety of the ship or completeness of its equipment. The Master shall report to the Administration...Recognized Organization responsible for issuing the relevant certificate who shall cause investigations to be initiated to determine whether a survey, as required by regulations 7,8,9 or 10 is necessary. Currently the vessel experienced an intermittent loss of pitch control inbound the port of Houston. A Class report/Survey of completed repairs will be required to clear deficiency. Shift to City Docks Dock 42 with 02 tugs as discussed, is allowed. SOLAS 74(2014 Cons Ed)/1/11(c) 60(a)(c).
