A pre-engineered fix gas fire extinguishing system must.....automaticially shut down all power ventilation systems and all engines that draw intake air from within the protected space. Upon testing, the system only secured the STBD main engine. The vessel rep identified a faulty relay in the system. MI witnessed proper operation.

Approx. 45 cracks and fractures were identified, localized around four adjacent frame bays amidships. The failures included one through thickness fracture of the hull. Numerous "v + weld" repairs noted in same general area. Vessel history indicates significant failures in same location caused by the normal working of the vessel. Observations and history call into question the fitness of the hull structure for the intended route and service. Produce a report from naval architect as to the adequacy of hull design for intended route and service.
