Engine room forward bulkhead has an opening on the overhead where the electrical cables are running through that is fill with absorbent mat pads, a combustible material.
Vessel shall remove absorbent mat pads and fill the hole with a non-combustible material that maintains the water integrity and fire boundary of the space.

The following systems must be equipped with gauges at the machinery location: main engine lubricating oil pressure and main engine RPM; main engine cooling water temperature; auxiliary generator engine lubricating oil pressure and auxiliary generator engine RPM; auxiliary generator engine cooling water temperature; and hydraulic steering fluid pressure, if the vessel is equipped with hydraulic steering systems.
RPM gauges for the main engines and the generator in the engine room are missing. Also, only one set of panel gauges is on the port side of the vessel with indication for jacket water temperature, and lube oil pressure, instead of having one panel for each engine (i.e. generator, and both main engines). Hydraulic pressure gauge as you enter the space is cracked.
Vessel shall install a cooling water temperature gauge, a lube oil pressure gauge, and RPM gauge for each engine operating inside the engine room space. Additionally, hydraulic pressure gauge as you enter the space shall be replace with a new one.
