Due to the RUTH being submerged, the vessel must identify the cause of the water ingress, make satisfactory repairs and undergo an inspection to the satisfaction of an attending Coast Guard Marine Inspector prior to movement or service.

Each exhaust pipe from an internal combustion engine which is within reach of personnel must be insulated or otherwise guarded to prevent burns. There was no insulation installed on either the Port/STDB inside engine exhaust manifolds located in the engine room. Install an adequate amount of insulation to protect personnel from burns while in the engine room.

Each towing vessel must be fitted with a general alarm that is activated at each operating station and can notify persons onboard in the event of an emergency in an engine room. In addition, install a supplemental flashing red light that is identified with a sign that reads: "Attention General Alarm-When Alarm Sounds or Flashes Go to Your Station." The vessel does not have a general alarm. Install a general alarm system capable of meeting the requirements of 46 CFR 142.235.

he portable fire pump must be self priming and power-driven with a minimum capacity of at least 80gpm at a discharge gauge pressure of not less that 60psi, measured at the pump discharge. The portable fire pump did not have an installed pressure gauge on the discharge. Install a pressure gauge on the discharge outlet of the fire pump.

All owners and operators of an existing towing vessel must have a valid COI prior to the specified phase-in period. This vessel is one of the 50% of the vessels that has not obtained a valid COI. The vessel shall be made available for a Coast Guard COI inspection within 90 days. If an inspection is not completed prior to this date, the action for this deficiency shall be changed to a 60 (Rectify deficiencies prior to movement) and the vessel may no longer be permitted to operate. 46 CFR 136.202(a)
