Cargo tanks in which Grade B or C liquids that are to be transported shall be fitted with either: individual pressure-vacuum relief valves or; a venting system consisting of branch vent lines connected to a vent header.. Cargo tank safety relief valves shall be inspected at least once in every 2 years. The safety relief valve discs must be lifted from their seats in the presence of a marine inspector by either liquid, gas, or vapor pressure at least once every 5 years to determine the accuracy of adjustment and, if necessary, must be reset. The relief valves for the two methanol tanks did not have any records of being tested since 2017. 46 CFR 32.55-20(c); 38.25-10(a); 38.25-10(b).

No OSV that carries flammable or combustible liquid with a flashpoint of below 140 degrees Fahrenheit may have electrical equipment installed within 10 feet of a source of vapor on a weather deck unless the the equipment is explosion-proof or intrinsically safe under 46 CFR 111.105-9 or 111.105-11. The junction box in the vicinity of the methanol pump did not meet the requirements of 46 CFR 111.105-9 or 111.105-11. 46 CFR 129.250(a).

If the periodic reveals deficiencies in vsl maintenance, all repairs must be made to the satisfaction of the [name withheld] for port side hydraulic filter was loose and leaking.

Each vsl must be designed and constructed to minimize fire hazards. Starboard main diesel engine had leaks from common fuel rail.

Each ring buoy must be properly marked with retroflective tape. Retro tape for port side ring buoy was faded.

EACH WATERTIGHT DOOR MUST BE DESIGNED TO WITHSTAND WATER OF HEAD PRESSURE EQUIVALENT TO 10 FT; OBSERVED LIGHT COMING THROUGH QAWTD WHEN DOGGED TIGHT; MAKE ADJUSTMENT OR REPLACE SEAL TO MAKE HATCH ON BACK DECK LEADING TO ENGINEROOM WATER TIGHT.

PRESSURE-INDICATING GAUGES ON PV'S SHALL DISPLAY 1.2 TIMES NOR MORE THAN 2 TIMES PRESSURE AT WHICH RELIEVING DEVICE IS SET TO FUNCTION; VSL HAD MAWP OF 200 AND NEEDED A GAUGE TO DISPLAY A HIGHER PARAMETER ON THE GAUGE; REPLACE GUAGE TO INDICATE PROPER LIMITS PER ABOVE LISTED REGULATION.

Fuel and hydraulic hoses shall be kept in serviceable condition. Replace wasted fuel and hydraulic hoses in the following locations: port and starboard steering gear hydraulic hoses inside the engine room, bow thruster fuel supply hose, starboard and port generator fuel hoses, port main diesel engine fuel return hose.

Armored electrical cables on the starboard engine were chaffed. Replace damaged armored cables.

Interlock for generators non functioning.

#1 & #2 generator did not have functioning overspeed device.

Repair fuel leak on bow thruster engine filters.

Replace bow thruster room CO2 extinguisher hose.

Replace stbd reduction gear cooling hose.

Provide proper operation of the 120v ground detector.

provide emergency steering insructions on the bridge and engine room.

Replace port steering pump hoses.

Remove oily water from engine room bilge.

Replace port SSDG fuel hose.

Tighten shaft packing on liquid mud pump.

Remove two (02) patches on bilge pump and make proper repairs.

Make engine room passageway bilge alarm indicator operable.

Replace all expired floating light batteries.

Replace cutoff valve at fire station number 2 on the main back deck.

Crop/Renew damaged side shell (inset) and framing Port Bow above main deck at #2 Ballast Tank at next drydock NLT 31DEC2017

Found a fire station with a charged hose in the rack. Identified that the valve was leaking by. Repair/Replace the valve.
