74 SOLAS (2001) Chapter II-1, Regulation 26.1: The machinery, boilers, and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which it they are intended and shall be so installed and protected as to reduce to a minimum danger to persons on board. The number 1, 2, and 3 generator is leaking fuel and oil that’s pooled underneath the engine and leaking down to the lower level of the engine room. The excessive leaks causes an unsafe condition and a fire hazard. Prior to vessel’s departure from COTP Guam, remove fuel and oil pooled underneath the generators.

Large plastic container used to store first aid kit, flash light with extra batteries and other equipment required per LSA Code was inspected with water inside. Rescue boat must be fitted with contents per LSA Code Chapter 5/5.1.2.1 and 5.1.2.2 and shall be as small and of as little mass as possible and shall be packed in suitable and compact form. Replace large plastic non-watertight container with a smaller watertight container to prevent contents and equipment from water damage or store in rescue boat storage locker or compartments.

74 SOLAS (2001) Chapter II-2, Regulation 13.1.13: The function of the detection system shall be periodically tested to the satisfaction of the Administration by means of equipment producing hot air at the appropriate temperature, or smoke or aerosol particles having the appropriate range of density or particle size, or other phenomena associated with incipient fires to which the detector is designed to respond. All detectors shall be of a type such that they can be tested for correct operation and restored to normal surveillance without the renewal of any component. The vessel does not have onboard the approved type of testing equipment for flame detectors. The testing has been conducted with an open flame which is not as per the manufacture's instruction and hazardous in a machinery space.

The main fire, bilge, and ballast pumps #1 and #2 were found to isolated due to excessive leaking seals, resulting in ingress of water into the bilge. The crew did not communicate to the company that isolation valves were closed, requiring additional time to align in case of emergencies.
