Provide 1 life jacket and immersion at watch station on Bridge.

The master must ensure that all hatches, doors, and other openings designed to be watertight or weather-tight function properly. The aft stern void hatch is inoperable and the forward hatch gasket material is dry-rotted. Make aft hatch operable and replace gasket on forward hatch. 46 CFR 140.610(b)

Decks and bulkheads designed to be watertight must be maintained in that condition. Severe wastage at the base of the STBD stack, approx 2'x1', and at the base of the port WTD, approx 1' x 6", was discovered. Provide plan to crop and renew back to good metal for the aforementioned areas prior to next operating season. 46 CFR 140.610(f) 24OCT2023: The vessel has been hauled out of the water for the winter season. The vessel rep will be assessing scope of repair work during this haul out period and submitting repair proposal. Repair work will be conducted in spring 2024 after review and approval of the proposal. Vessel rep will request attendance for verification of repair work prior to placing the vessel back into the water around the beginning of June.

High bilge level alarms must be visible and audible at each operating station. The bilge high level alarm did not provide adequate audible notification at the operating station. Install adequate audible alarm at operating station. 46 CFR 143.230(b)(1)

Each towing vessel must be equipped with navigation lights in accordance with the International Regulations for Prevention of Collisions at Sea (COLREGS) or 33 CFR part 84. The port side running LED light is partially obstructed and not providing the required horizontal arc of visibility, install proper navigational light. 46 CFR 140.720

The master or officer in charge of a navigational watch must determine whether the vessel complies with all stability requirements in the vessel's stability letter. Bilges were found with several inches of standing oily water, pump bilges and provide documentation of removal in TVR. 46 CFR 140.605(a)

Each entrance to a space storing carbon dioxide cylinders, a space protected by carbon dioxide systems, or any space into which carbon dioxide might migrate must be conspicuously marked: Spaces protected by carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. WHEN ALARM OPERATES, DO NOT ENTER UNTIL VENTILATED. LOCK OUT SYSTEM WHEN SERVICING.” Spaces storing carbon dioxide—“CARBON DIOXIDE GAS CAN CAUSE INJURY OR DEATH. VENTILATE THE AREA BEFORE ENTERING. A HIGH CONCENTRATION CAN OCCUR IN THIS AREA AND CAN CAUSE SUFFOCATION.” The machinery space and forward storage space were Carbon Dioxide is stored were not marked, properly mark entrances to spaces. 46 CFR 136.110 46 CFR 78.47-11(b)-(c)

All piping, valves, and fittings shall be securely supported, and where necessary, protected against injury. The CO2 branch line in the centerline overhead of the engine room had a broken support bracket and not properly supported. Repair support bracket. 46 CFR 136.110 46 CFR 76.15-15(f)

Each escape hatch and emergency exit used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”. The emergency exit from forward engine room was not marked, mark emergency exit. 46 CFR 144.160(f)

Each watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”. The watertight hatches and doors to the lazerette, machinery space, house, and forward storage space were not marked. Properly mark watertight hatches and doors. 46 CFR 140.160(e)

The master of a towing vessel must ensure that each crewmember participates in fire-fighting drills and receives instruction at least once each month. Fire drills have not been conducted since and recorded since June 30, 2023. 46 CFR 142.245(a)

Vessel must have a type I Lifejacket for every person onboard. Vessel currently only has 02 lifejackets.

Fire pump must deliver 50psi and be energized remotely from a safe place outside the engine room. Fire pump does not deliver 50 psi to hydrant and cannot be energized from outside the engine room.

The design and installation of the fire detection system must be certified by a professional engineer or NICET Level 4 alarm engineering technician. Installation is not certified.

All installed bilge piping must have check valves installed to prevent backflooding.
