A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space; and a means to reset devices used to automatically shut down ventilation systems and engines as required by this section. During testing of the fireboy system shutdown/ override, the main engines did not shutdown after the wire(s) were disconnected from the bottle and subsequently, MI was unable to verify the fireboy override system. 46 CFR 181.420

Replace port and starboard shaft cutlass bearings total of (04) worn past 50%. Vessel name and homeport not painted on the stern. Loading marks not painted contrasting colors. Port and starboard main engines missing air filters. Replace all steering hoses and fittings in the lazarette because of worn condition. Clean bilges of oil, dirt and salt deposits for the following spaces: engine room, lazarette, and #2 void for reinspection. While the vessel was out of service, it was repowered with Scania main engines. Provide spec sheets for new engines, old engines and submit the repowering guide provided to you by this office.

The owner, managing operator or master shall report damage to machinery that renders further use of the item unsafe until repairs are made, to the OCMI at the port in which the causality occurred as soon as practicable after the damage occurs. MSD St. [name withheld] received a notification 5 days after the occurrence of the starboard engine cooling system failure. Prior to the operation of the vessel attending Coast Guard Inspectors will witness repairs and sea trials of the vessel.
