...The master or person in charge of an oceangoing ship that cannot discharge NLS residue into the sea in accordance with 46 CFR 153.1126 or 153.1128 shall ensure that the NLS residue is—(1) Retained on board; or(2) Discharged to a reception facility. The methanol containment around the loading and discharge manifolds allows spills on the deck to flow directly into the sea. Four sided containment must be properly installed around the manifolds prior to carriage of methanol (methyl alcohol). If the vessel is currently carrying methanol, a one-time discharge is authorized to clean and gas free tanks. After request for reconsideration was rescinded, MSU Houma agreed to extend the 835 until the vessel's next scheduled drydock in Nov 2026.

In meeting these requirements the company should ensure that any non-conformity is reported with its possible cause. The vessel reported a failure of the emergencny generator to the company on 7/18/2025 which lasted until USCG attendance on 11/13/2025. A report was never made to the USCG. Conduct root cause analysis of non-conformity.

Machinery spaces of category A containing oil fuel units shall be provided with one of the systems in para 4.1. The CO2 flex hoses were loose and unsealed to the bottle on 5 of the bottles. 74 SOLAS (20) II-2/10.5.1.1

Electrical installations shall be such as to minimize the risk of fire from flam. products. Install certified safe lights in cargo deck haz zone. Recitfy prior to methanol carriage. 74 SOLAS (20) VII/10.1, IBC 10.1.2.1

Oil piping drains, strainers, and other equip. subject to normal leakage must be fitted with drip pans. The E-gen prime mover is missing drip pans under the fuel filters and fuel sample port. 46 CFR 56.50-60(k)

Each vessel must have a means of comms among the following: steering gear room. The light on the sound powered phone beacon was extinguished. 46 CFR 113.30-5(a)(2)

Electrical installations on vessels must ensure: protection of passengers, crew, other persons, and the vessel from electrical hazards. 01 breaker panel in the machinery space had excessive oil within the panel, another breaker panel was missing a blank and was open to contact.

Vent outlets from all tanks which may emit flammable or combustible vapors, must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh spaced not less than one-half inch (13mm) nor more than 1 1/2 inches (38mm) apart. STBD Fuel vent flame screen was not secured to the vent.

When the vessel is underway, the EPIRB must be mounted in a manner so that it will float-free if the vessel sinks. EPRIB bracket was not made for the type of EPIRB carried by the vessel and could not be properly secured in it's bracket.

Design Verification and Periodic Safety test procedure documents approved by the Coast Guard must be retained aboard the vessel. Periodic Safety test procedures onboard were not CG approved. Provide CG approval within 90 days.

Each cargo vessel must carry at least one rescue boat. Each rescue boat must be approved under approval series 160.156 and be equipped as specified in table 199.175 of this part. Vessel was not in possession of Certificate of Approval for their rescue boat.

On U.S. inspected ships, oily water separating equipment and bilge alarms must be approved under 46 CFR 162.050. Vessel was not in possession of Certificate of Approval for oily water separator.

Each fire hydrant shall be identified in red letters and figures at least two inches high “FIRE STATION NO.#", Fire station on starboard aft main deck was not marked.

Each side of each rescue boat bow must be marked in block capital letters and numbers with the name of the vessel and the name of the port required to be marked on the stern of the vessel to meet the requirements of subpart 67.123 of this chapter. Rescue boat was not marked with vessel name and hailing port.

The revised NOx Technical Code 2008 shall be applied in the certification, testing, and measurement procedures for the standards set forth in this regulation. There was no documented evidence of NOx influencing engine component replacements/repair in the vessel’s Engine Technical Files.

The signal at both EOT locations must audible and sound continuously when the transmitter and the indicator do not show the same order; EOT at e-steering was not audible.

Each vessel must meet the applicable requirements in Subchapter S; escape hatch from e-steering was not sealing appropriately.

E-gen shall be provided with two means of energy for starting; battery bank for starting was not separated into two means.

No repairs or alterations to machinery that affects the safety of the OSV may be made without notice to the OCMI. On 13NOV23 MSU Houma received notification that the vessel's forward main generator was taken out of service due to a bad lead wire. Provide a technician's report attesting to the satisfactory operation of the vessel's forward main generator. 46 CFR 126.150

No repairs or alterations to the machinery may be made without notice to the OCMI. Vessel experienced fire as a result of wiring failure on the generator for the main engine. Vessel must make proper repairs to wiring prior to departure.

The auxiliary steering gear must be— 2) Capable of moving the rudder from 15° on either side to 15° on the other in not more than 60 seconds. The pumps powered up but would not actuate the steering gear. Repair and test.

(b) Where a generator driven by a propulsion engine is used as a source of electrical power, no speed change, throttle movement, or change in direction of the propeller shaft of the vessel may interrupt power to any of the loads specified in paragraph (a)(1) of this section. The E-gen continuously tripped off line while supporting the emergency electrical load.

Except as provided by paragraphs (l) and (m) of this section, there must be at least two means of escape, exclusive of windows and portholes, from each of the following spaces: (1) Each space accessible to offshore workers. (2) Crew accommodations and each space where the crew may normally be employed. The aft steering escape hatch was frozen closed and could not be opened from the space.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary must be at least equivalent to the standards established by the ABS Steel Vessel Rules. There was a hydraulic leak on the bow tunnel thruster. There was also a hydraulic leak and broken hydraulic line bracket on the swing down thruster. All hydraulic leaks are required to be corrected.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary must be at least equivalent to the standards established by the ABS Steel Vessel Rules. There was an oil leak in the starboard main diesel engine and oil leak in the starboard shaft generator reduction gear. All hydraulic leaks are required to be corrected.

Electrical installations on vessel must ensure protection of the vessel from electrical hazard. Steering gear motor controller #3 had a broken main switch. Switch is required to be fixed for proper operation.

Vessel is 3,045 ITC and multi-certificated under 46 CFR subchapters L & I. IAW 46 CFR 62.01-5(a), self propelled vessels of 500 GT and over that are certificated under subchapter I are subject to 46 CFR Part 62, Vital System Automation under subchapter F. As per 46 CFR 62.20-1, design verification and periodic safety test procedures are required and must be submitted to the Coast Guard for approval.

Inspectors observed defective fire hoses during operational test of fire main to leak test all fire hoses onboard. Master shall ensure that the vessel's firefighting equipment is at all times ready for use and equipment is maintained & replaced as indicated.

Electrical installations in hazardous locations must comply with the standards listed in 46 CFR 111.106-3 for intrinsically safe or explosion-proof installations. Inspectors observed two (02) lights on the aft deck by fuel vents & cargo vents with improper wiring for the hazardous location.

Provide proper operation of emergency fire pump and ability to produce 50 PSI from every hydrant independently.

Provide two independent fire pumps each capable of simultaneously producing approximately 50 PSI measured by pito tube out of 2, 2.5" hoses from the 2 highest fire main hydrants onboard. Note** this is a correction to the had written item #2 on the CG-835 sent via email .

Door was missing CO2 Storage placcard, required vessel rep to provide CO2 storage placcard IAW 46CFR97.37-11

Emergency Fire Pump operated at pressure below required 50 psi. Actual pressure was 10 psi at the pump.

Prove proper operation of PA system interrupter for the fire/general alarm prior to 14DEC07 or prior to any international voyage, whichever comes first.

Provide a minimum one bbl capacity fixed containment under the emergency generator fuel vent at the next drydock period.
