At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected. MI observed the sewage suction line connection to the through hull fitting in the forward port side engine room was leaking. Prior to final rectification, submit a remediation proposal to the attending Marine Inspector for approval.

Vessel described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI observed missing detector located within pilot house. Properly install fire detector within pilot house to meet regulations outlined above. 46 CFR 181.405(c), Code 16.

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. Port dry exhaust piping routed through the steering compartment has exposed sections of the piping. 46 CFR 177.97, code 16.

Means of escape: The two required means of escape must be widely separated and, if possible, at opposite ends or sides of the space to minimize the possibility of one incident blocking both escapes. Vessel has a secondary door in front of the FWD engine room door that is lockable only from the outside. 46 CFR 177.500(b), code 16.

Electrical equipment on a vessel must be installed and maintained. Two electrical junction boxes were observed without proper closure and one 110v outlet was found with missing cover and contaminated with oil. 46CFR183.200

A bilge suction must be fitted with a suitable strainer. Multiple suction points were found without strainer. 46CFR182.510(b)

Machinery spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. A hole was observed in the overhead above the #2MDE. 46CFR177.405(c)

Each motor must have a nameplate attached to it containing the information required by Article 430 of NFPA 70. Generator did not have nameplate. 46CFR183.320(d)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. MI observed that the vessel only had one detector in the forward passenger space. Vessel is to install an interconnected fire detection system that covers each of the following spaces: lazarette, machinery space, forward passenger space, and wheelhouse. 46 CFR 181.405(c)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. MI observed the generator had multiple fuel leaks between the fuel pump and high pressure fuel lines. MI observed fuel pump replaced and satisfactory operation on the generator. 46 CFR 182.100

Each item of survival craft equipment must be of good quality, and efficient for the purpose it is intended to serve. Both of the onboard lifefloats were deteriorated beyond their service life. Both were replaced and installation was witnessed by MI. 46 CFR 180.175 (a)

Except as otherwise allowed by this subpart, a vessel must comply with the structural design requirements of ABS Steel Vessel Rules, (<61 Meters), with the appropriate conversions from the ABS Aluminum Vessel Rules. There is an approximately 2' X 1' area of localized pitting on the hull plating next to the fuel tank as well as an approximately 2' x 1' area of localized pitting along the bottom on the port side of the vessel's fuel tank. Submit repair plan and replace in kind or provide gauging report from a [name withheld] attesting to allowable wastage of the tank and surrounding hull plating at vessel's next dry dock. 46 CFR 177.300 705 C

Vessel found to be operating without a valid COI. Anniversary date is 21AUG and vessel continued to operated after 21NOV18. Vessel to cease operations until new COI inspection has been completed and given a temporary COI. 46 CFR 176.502 60c

Provide repair proposal for port side fore peak damage to include the collision bulkhead and engine room aft port side damage to Sector New Orleans Domestic Inspections prior to conducting repairs and carrying passengers.

Supervisor nor the master of the vsl had received and stated that there was not a drug awareness training program in place as required by 46 CFR 16.401 (b)

Cover battery negative bus bar terminals throughout the engine room space.

Properly install VHF radio antenna (antenna bracket is broken)

STBD main propulsion engine is inoperable. Prove proper operation to the attending Marine Inspector or fax a copy of the tech repair report prior to sailing

Prior to carrying passengers conduct new to zone inspection and underway man overboard and fire drills.

Prove enrollment in an approved drug and alcohol program.

Prior to carrying passengers provide appropriate log books, navigation charts and navigation publications for operating area.
