Machinery,pressure vessels, and associated piping systems shall be of design and construction adequate for intended service and shall be so installed to reduce to a minimum any danger to persons on board, due regard being paid to moving parts. PSCO observed low temperature fresh water pump's moving parts exposed to a walkway. Driveshafts were partially guarded by temporary plastic barriers.

PSCO observed a ratchet strap installed as a temporary repair to prevent against vibration of the low pressure CO2 system.

Stairways and lift trunks which penetrate more than a single deck shall be surrounded by at least "A-0" class divisions and be protected by self-closing doors at all levels. PSCO observed the fire door for galley, bridge, deck 06 engine room door, and deck 11 incinerator room door were not self closing.
