The design, materials, and construction of masts, posts, yards, booms, bowsprits, and standing rigging on a sailing vessel must be suitable for the intended service. Age of standing rigging is unknown. Provide Objective evidence of rigging age as well as proposed replacement dates. Add to current PMP. 46 CFR 177.330

Multiple lock nuts for turnbuckles and toggle pin nuts were loose on port and stbd shroud terminal gear at deck. Write inspection points for these items into current PMP. 46 CFR 177.330

Control systems for class 3 watertight doors must have power supplies, power sources, installation tests and inspection, and additional remote operating consoles in accordance with Supplemental Requirements Nos. S1 through S4 of ASTM F 1197. Class 3 watertight doors do not operate on battery backup. 1. Maximum of 60 passengers and 10 crew. 2. Watertight doors are to remain closed at all times (no transiting). 3. Vessel must remain on generator power while underway. 4. Passengers must remain on the weather deck except for when using the restroom, and return to the main deck immediately afterwards (no congregating in accommodation space).

Fully pull broken keel bolt stbd side frame 26, and pull port keel bolt in frame 25. Frame 26, nut sheared at the bronze floor. Complete pull and inspection prior to credit dry dock.

Replace stem on valve for port side generator exhaust water. Prior to credit dry dock.

Vessel operator has made unapproved changes to electrical installations onboard vessel. Vessel must have electrical system plans approved by MSC and confirmation of installation / operation by cognizant OCMI. 46 CFR 176.700(a)

Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel does not have correct breakers isolating 240v breaker panels. 46 CFR 183.310(b)

Passengers must remain on the weather deck except for when using the restroom, and return to the main deck immediately afterwards (no congregating in accommodation space).

For fixed-gas fire extinguishing systems...The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. MI found gauge on actuator bottle to fixed FM200 system indicating empty. 46CFR176.810(b)(2)

Machinery and fuel tank spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. MI observed circular hole approx. 7" diameter in aft bulkhead leading into aft head. 46CFR177.405(c)

Vessel’s aft watertight bulkhead was found wasted in multiple spots. Aft bulkhead requires final weld NDT. 46CFR176.700(a)

When required by a marine inspector, a portion of the inspection must be conducted while the vessel is underway so that the hull and internal structure can be observed, including the condition of all spars, standing and running rigging, clocks and sails. Vessel was unable to let out the main and fore top sails. Prior to the carriage of passengers with the main and fore top sails set, attending MI’s must inspect set sails while underway.

At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. MI did not witness drill completed on annual inspection with minimum required crew IAW manning on COI. To be completed along with def #4.

The number and identification of the items stowed inside, and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Container of PFDs inside main saloon was not marked appropriately. 46CFR185.604(f)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Adult and child PFDs were mixed together inside container located in main saloon. 46CFR180.78(a)(5)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Light Light, Charts, and Coast Pilot [name withheld], charts, and Coast Pilot were outdated. 46CFR184.420(a)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. For equivalent kits, the contents must be stowed in a suitable, watertight container that is marked “First-Aid Kit”. A first-aid kit must be easily visible and readily available to the crew. First aid kit was found not in a watertight container and did not meet required contents. 46CFR164.710

Sliding watertight doors must meet the requirements of part 170, subpart H in subchapter S of this chapter. Aft WTD was not operating as designed. 46CFR179.330

Piping, including valves, pipe fittings and flanges, conveying vapor, gas, or liquid, the temperature of which exceeds 65.5 °C (150 °F), must be suitably insulated where necessary to prevent injuries. MI observed lagging on port main engine turbo ripped and disintegrating. 46CFR177.970

Master reported the top deck of the vessel had multiple cracks and the plywood beneath was rotted in multiple areas. Submit plans to repair as original to local OCMI. 46CFR176.700(a)

Vessel operator is currently in process of removing unauthorized electrical installations that were previously installed at unknown time. Vessel is required to submit all electrical repair proposals to MSC. 46CFR176.700(a)

Vessel does not have complete inventory of sail equipment logged in their preventative maintenance plan and thus has not created a maintenance/examination schedule for each component. PMP is partially drafted. Provide completed PMP for all sailing equipment. 46CFR177.330

WTD visual indicators did not illuminate during operational test. Vessel has experienced multiple electrical issues due to previous unauthorized repairs. Vessel's aft water tight bulkhead was found wasted in multiple spots. Vessel was permitted to complete 2022 season with temporary repairs. Submit plans to replace as original to local OCMI. 46CFR176.700(a)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. Vessel is required to complete material inspections on all rigging turnbuckles prior to sailing with sails set with passengers. 46CFR176.840

Due to location of vessel, we were unable to get underway to conduct drills. Conduct drills in the presence of a marine inspector prior to 10/28/2023.

Due to location of vessel, we were unable to get underway to observe rigging and sails under normal operating conditions. Conduct underway inspection of rigging and sails in the presence of a marine inspector prior to 10/28/2023.

Vessel began process of adding bow thruster without approved plans through MSC. Submit plans to MSC for approval and provide local OCMI with submission report. 46CFR176.700(a)

46CFR176.700(a): Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] plank/seam on starboard side.

46 CFR 176.808(g)/46 CFR 176.810(d): At each initial and subsequent inspection for certification, the marine inspector may require that an abandon ship or man overboard and fire drill be held under simulated emergency conditions specified by the inspector. Due to COVID-19 and crew restrictions, the vessel could not get underway to conduct drills.

46 CFR 176.810(a)(4): Testing of each fire hose to a test pressure equivalent to its maximum service pressure. Both fire hoses were found with exterior material damage and must be replaced.

46 CFR 185.420(c): Crew training shall be logged or otherwise documented for review by the Coast Guard upon request. Vessel did not have its logbook onboard.

Vessel has pulled masts for winter maintenance and repairs. Prior to carry passengers, complete mast step inspection, mast repair inspection, an operation test and sea trials to the satisfaction of the attending CGMI.

Endorsements on Certificate of Documentation (COD) are valid for one year. Vessel owner did not have current COD on board. Provide current copy of COD to attending Coast Guard Marine Inspector. (46 CFR 67.163 (a))

Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Vessel owner did not have life jacket donning instruction placards located in conspicuous place on board the vessel. Provide donning instruction placards to the attending Coast Guard Marine Inspector.

The master and crew of vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of a fire. Vessel owner did not have operating procedures at the fire activation station. Provide emergency procedures to the attending Coast Guard Marine Inspector.

The vessel is currently broadcasting (1) Improper Name, (2) Zero Length and Beam, (3) No Ship Type

Vessel does not have primary lifesaving onboard. Vessel is restricted to a LBS no more than 1 mile from shore . Vessel must submit a proposal for replacing liferafts or permanently changing the route on the vessel's COI.

Vessel does not have required primary lifesaving onboard. During warm water months vessel is restricted to within Limited Coastwise, LBS, & 20 miles of Harbor of Safe Refuge. No extensions of this CG-835 will be permitted.

Vessel does not have required primary lifesaving onboard. Vessel is restricted to within one mile of shore until required lifesaving is provided.

Vessel only has 53 serviceable adult life jackets onboard. Vessel is restricted to 53 persons onboard total until the number of life jackets is increased.
