Electrical equipment and installations must be suitable for the roll, pitch, and vibration of the vessel underway. MI observed dead end wiring from previously installed water heater. Remove dead end wiring to the satisfaction of the attending MI.

Except as allowed by paragraph (d) of this section, each vessel constructed of a material other than wood certificated to operate on a limited coastwise route in cold water must be provided with life floats of an aggregate capacity that will accommodate at least 100% of the total number of persons permitted on board. Marine inspector (MI) observed the 12-person life float was deteriorated making it not fit for service. Vessel is restricted to a maximum of 22 persons on board.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. MI found hydrostatic release unit for EPIRB was expired. Vessel is restricted to operating within 3 NM from the coastline until hydrostatic release unit for EPIRB is replaced.

All vessels must have navigation lights that are in compliance with the applicable sections of the International and Inland Navigation Rules. MI found masthead light and both port and starboard navigation lights non-operational. Vessel is restricted to daytime operations until all navigation lights are repaired.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. MI observed the COD on board is expired.

Employers must maintain records of chemical tests as provided in 49 CFR 40.333 and must make these records available to Coast Guard officials upon request. MI observed the vessel did not have a current MIS report from their drug testing consortium.

The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. MI observed there was no firefighting servicing certificate on board.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: A light to indicate discharge. Indicator light for system status "Charged" was not illuminated.

A pre-engineered fixed gas fire extinguishing system must: Be capable of manual actuation from outside the space in addition to automatic actuation by a heat detector; Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Pre-engineered fixed gas fire extinguishing system for engine room was inoperable. System display status indicator was found without power and when system was tested was unable to secure engine or make notification to operator if system was deployed. 46 CFR 181.420(a)

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the normally unmanned spaces. Visual indicator was found inoperable for the house space bilge. 46 CFR 182.530(a) Action code: 705: rectify within 7 days.

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers, other than a ferry, provided the pump is permanently mounted to bilge. Submersible bilge pump in house bilge space was found detached from hull. 46CFR182.520(e)(3)

A flexible tube or hose may be used instead of fixed pipe for the discharge line of a submersible electric bilge pump provided the hose or tube does not penetrate any required watertight bulkheads and is of good quality and of substantial construction, suitable for the intended use; and highly resistant to salt water, petroleum oil, heat, and vibration. Flexible bilge hose for submersible pump in engine room was found wasted. 46 CFR 182.520 (f)

Each small battery installation must be located in a well ventilated space and protected from falling objects. House batteries in house bilge space were found with exposed battery terminals.46 CFR 183.354(b)

Overcurrent protection must be provided for each ungrounded conductor for the purpose of opening the electric circuit if the current reaches a value that causes an excessive or dangerous temperature in the conductor or conductor insulation. Multiple connections were found on house batteries located in house bilge space without any overcurrent protection and without approval or oversight from Coast Guard. All power supply for electrical conductors shall be from the vessel’s distribution panel with properly sized overcurrent protection per the vessels approved electrical plans. If installed distribution panel is unable to support accessiories installed onboard, submit plans to add an additional electrical distribution panel. 46CFR183.380(a)

If individual wires, rather than cable, are used in systems greater than 50 volts, the wire must be in conduit. Electrical pigtail from inverter in house bugle space was found installed in a manner that exposes crew to an electrical shock hazard. Electrical wires shall be in conduct and an electrical receptacle box with cover; or properly terminate wires and discontinue use. 46CFR183.340(a)

Cable and wire for power and lighting circuits must: be listed by Underwriters Laboratories (UL), as UL Boat or UL Marine cable; and be be No. 14 American Wire Gauge (AWG) or larger. Conductors in control and indicator circuits must be No. 22 AWG or larger. Multiple conductors in house bilge space were found to be not designed for marine use and of an inadequate size (AWG). 46CFR183.340(d)

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Closure devices shall be a material that is not combustible and can be mounted near the supply ducts without being destroyed from the marine environment. 46CFR182.465(h)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. Ring buoy was missing retroreflective material; 4 strips on each side and evenly spaced apart. 46CFR185.604(i)

Oceans, coastwise, limited coastwise, and Great Lakes routes. A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry: Six hand red flare distress signals approved in accordance with §160.021 in subchapter Q of this chapter, and Six hand orange smoke distress signals approved in accordance with §160.037 in subchapter Q of this chapter, or 12 red flare distress signals total. Vessel was found with 11 red flare distress signals; crew sent to purchase additional flare. 46CFR180.68(a)

For each marine casualty required to be reported by § 185.202, the owner, agent, master, or person in charge of the vessel shall determine whether there is any evidence of alcohol or drug use by individuals directly involved in the casualty. Expired alcohol testing equipment was found expired, minimum of 02 tests are required on board. 46CFR185.210

A power-driven vessel underway shall exhibit: A masthead light forward; Masthead light means a white light placed over the fore and aft centerline of the vessel showing an unbroken light over an arc of the horizon of 225 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on either side of the vessel. Vessels masthead light was found extinguished, vessel restricted from operating during periods of restricted visibility. 33 CFR 83.23

The owner, managing operator, or a qualified servicing facility as applicable must conduct the following inspections and tests: annual maintenance and recharging of extinguishers. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Inspection tags on all fire protection equipment noted the last servicing was performed in April 2022.

On a vessel of at least 26 feet in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in a machinery space bilge, bilge well, shaft alley bilge, or other spaces subject to flooding from sea water piping within the space. The high level alarm for the main space does not provide a visual notification and the engine space provides a visual notification for the wrong location.

A vessel of more than 26 feet in length, but not more than 65 feet, must carry a minimum of one life buoy of not less than 24 inches in diameter, approved in accordance with §160.050 in Subchapter Q of this chapter. The grab line was found to be deteriorated.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. The battery of the vessel's EPIRB expired in February 2022 and the NOAA registration expired in January 2021. The vessel may not transit more than three miles from the coastline with passengers until these requirements are met.

A vessel not required to have a power driven fire pump by §181.300 must have at least three 2.5 gallon buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. The vessel had only two fire buckets of the required size on board.

Alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant [name withheld] that affect the safety of the vessel include damage repair or replacement of electrical wiring and power supply systems. An extension cord was found connecting chargers in the lazarette to the anchor winch controls at the operating station. Submit electrical plans to the OCMI.

Except on a vessel limited to daytime operations, the magnetic compass must be illuminated. Magnetic compass illumination could not be proved.

Abandon ship, man overboard, and fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The drill entry shall include the date of the drill and training and a general description of the drill scenario and training topics. The vessel operator could not provide a log or documentation of drills and/or training.

A vessel must carry adequate and up-to-date U. S. Coast Pilot or similar publication and Coast Guard Light List. The publications found on board were the 1995 editions.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection in accordance with §80.59 once every five years. If the ship passes the inspection, the technician will issue a Communications Act Safety Radiotelephony Certificate. The certificate was found to be faded and illegible. CITE: 47 CFR 80.903

Employers must ensure that they and their crewmembers meet the requirements of this part (46 Part 16). The most recent MIS Report could not be produced.

A visual indicator must be provided at the operating station. Vessel did not have labels on their bilge alarm lights at the operating station.

Connections must be made to battery terminals with permanent type connections. Temporary type clamps are prohibited. Vessel had onboard wingnuts for their batteries in the fwd (house)compartments.

The float free link must be secured to the painter at one end and the vessel on the other end. Vessel painter was improperly attended to lifefloat grabline.

A vessel on an limited coastwise route must carry: six red hand flares and six hand orange smoke distress signals. Vessel did not have the correct number of flares onboard.

Due to COVID-19 circumstances beyond the control of the vessel, the vessel is unable to meet the timeframe required by 46 CFR 176.107. Vessel is allowed 90 days to continue operations in accordance with the COI, and is required to make itself available for inspection prior to 27JUN2020. Issued CG-835v code 705c to extend COI Inspection 90 days.

Provide means to completely close engine room ventilation, as space is protected by a fixed gas extinguishing system. Must be fixed prior to carrying passengers, IAW 46CFR182.465(h).
