0615: Before the ship leaves port and at all times during the voyage, all life saving appliances shall be in working order and ready for immediate use. Both port and starboard lifeboat coxswain seats are not bolted down, and are able to move freely creating a safety hazard within the lifeboats. The port lifeboat has numerous seats that are wasted which will not allow the crew to properly use safety belts. The starboard lifeboat has a patch on the hand rail above the forward hatch cover, 74 SOLAS (14) III/20.

2120: Every ship shall carry a garbage management plan which the crew shall follow. The garbage management plan is not being followed. Mixed garbage was found throughout the ship as well as at the final collection point prior to being discharged into the sea. The crew has disposed of food waste overboard 30 times which the PSCE believes included mixed garbage. Furthermore, there is an excessive amount of stored trash which is currently posing a health, safety, and environmental hazard. The trash shall be properly disposed of prior to departure from the San Francisco COTP Zone, MARPOL V/10.
