The Chief Engineer removed the International Shore connection from its intended location to connect the seawater pump and freshwater generator to replace a portion of wasted piping in the engine room. The Chief Engineer failed to notify the Master, Company, Class Society, or the Flag State. The vessel failed to fully implement the requirements of the SMS as evident by deficiencies 02, 04, 05, 07, 09, 12, and 13. An external audit is required. 74SOLAS (20) IX/3.1 ISM Code 2018 Part A 5.1

The machinery, boilers and other pressure vessels, associated piping systems, and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board due regard being paid to moving parts, hot surfaces and other hazards. After being manually shut down, the emergency generator will restart on its own with no known cause for its restart, posing a hazard to any persons that may be working on the prime mover at the time it automatically starts.74SOLAS(20) II-1/26.1
