Before the vessel leaves port and and all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Rescue boat #4 had a drop in pressure from its stored form of power system (accumulator). Port oar lock was cracked compromising the ability to seat the oar lock in place. Provide schedule of periodic maintenance checklists. Noted: Hull deflections caused by improper stowage, stress cracks and missing plugs were observed that do not impact the rescue boats operating condition will be subject to continued monitoring on follow-up USCG inspections and crew weekly checks.

Emergency Exit signs are required and must be installed IAW Sub-J. Exit sign did not have a cover, emergency exit signs that were discovered broken require renewal and red exit signs that are damaged require renewal. One bank of emergency lights was identified extinguished during the walk through.

All magnetically controlled fire doors shall be operated locally and by remote control. During the remote closure test one door was noted to have it's magnetic securing mechanism hanging requiring re-tightening. Complete within 7 days.

Lifesaving equipment must be in working order and ready for immediate use. Ring life buoy on hurricane deck had a missing plug and a water light line requiring renewal.

Each opening in an exposed weather deck must have a means for closing it weather-tight. The door leading into the Emergency Generator Space on the hurricane deck (04) is wasting at the bottom and is not weather-tight. Make weather-tight by 11 October 2023

In all ventilation systems, manually operated dampers or other suitable means shall be provided in accessible locations, outside the spaces served by the system, for shutting off the passage of air in the event of fire. The manual vent closure in way of the #2 rescue boat frame 72 is not able to close the louvers. Demonstrate that when the handle is in the closed position the louvers close, by 11 October 2023

All valves required in an emergency to operate to maintain its stability, and to operate the bilge systems must have a manual means of operation. Remote valve controls must be accessible under service conditions. Remote valve controls that are not readily identifiable as to service must be fitted with nameplates The Marine Inspector found that many of the remote manual operating valves have covers that are not easy to open, and the labeling is not easy to read. (The access are stamped but not easy to read). Demonstrate to the attending Marine Inspector that all the remote valves and access covers can be easily opened, operated easily identified as to their service. Specifically on the main deck passenger area and vehicle area. By 11 October 2023

Electric installations on vessels must ensure: Protection of passengers, crew, other persons, and the vessel from electrical hazards. In the Port Drive/Motor room, on the SI side, there is dead end wires exposed and hanging freely.

Each pendent-type fixture must be suspended by and supplied through a threaded, rigid conduit stem. In the Main Engine room there is an overhead light fixture secured and hanging by bungee cords.

The inspection of the fire panel to assure the safety of the vessel indicated a fault/alarm, one of which could not be reset by crew troubleshooting. Provide a technician report with a statement that correction actions have been made to clear the fault/alarm.

Vessel had an issue with the air dampner on the #2 SSDG, causing it to close and secure.

Vessel experienced a crankcase explosion in the #6 & #14 cylinders of the #2 Main Engine. Engine's casing, block, camshaft, pistons, connecting rods and cylinder liners are damaged. Vessel must determine the extent of damage & submit a repair proposal for approval. 46CFR71.55

After vessel makes repairs to the #2 main engine, vessel must demonstrate all safety alarms and shut downs to a Coast Guard Marine Inspector. 46CFR50.05-10

Public Address (PA) System was inoperable due to faulty computer/operating system.

Vessel is overdue for boiler mount inspections on both #1 and #2 Auxiliary Heating Watertube Boilers in the main engine room. Ensure boiler mounts are opened and examined to the satisfaction of a Coast Guard Marine Inspector prior to carrying passengers.

Vessel’s 4-Rescue Boat Davits have not been weight tested in the presence of a marine inspector after installation of new wire falls. Ensure davits are weight tested to the satisfaction of a Coast Guard Marine Inspector prior to carrying passengers.

#4 Drive had a faulty car. Vessel must repair or replace card and contact CG MI to witness operational test on the #4 drive.

Found several portable fire extinguishers boxes being used to store plastic bags, and spare parts, clean out all fire lockers. (Vessel representative was informed that this is a recurring problem and would be issued as a CG835 then cleared for documentation)

Hurricane deck fire door starboard side New York end is missing the sensor to show door is closed, replace the sensor.

During fire drill in snack bar it was discovered that the shutter doors leave a gap of 1" plus, CG835 issued to close the gap.

Demonstrate alarm and indication panel for closure of all fire doors.

Repair MDE to the satisfaction to attending Cg marine inspector.

CG835 issued Replaced faulty transformer for #1 Propulsion Drive and prove proper operation of same to the satisfaction of attending Marine Inspector. This requirement to be completed prier to placing vessel back in commercial passenger service.

Unsanitary conditions were found in the concession area. Clean area and have the concession area inspected by the NYC Health Department (DOH) no later than 15 May 2009 and provide inspection report that the concession area meets NYC DOH standards to the [name withheld] to this requirement will mean closure of the concession area.

Provide proof of sanitation inspection is being performed by vessel master at least once a month and documented as required by 46 CFR 71.45-1.

MDE's are tripping on reverse power. When problem has been identified and repairs made, provide a causal analysis report. Prior to carrying passengers demonstrate proper operation to the CG.

The vessel lost propulsion due to an encoder fault on the #3 drive and an encoder mismatch alarm on #4 drive. Provide technician's service report, and demonstrate proper operation to the CG.

While vessel was moored at Whitehall terminal the #2 Drive tripped off line causing the remaining 3 drives to also trip. Vessel is proceeding to troubleshoot the #2 Drive and transformer, provide a causal analysis report and make repairs . When repairs have been made demonstrate proper operation to the CG.

During test of power management system (PMS) stand-by engines failed to start automatically. Starter engaged, engine cranked, but would not start without manual manipulation of the fuel rack. Operator ordered to troubleshoot, take corrective action and test to satisfaction of attending marine inspector.

Provide tech report to determine underlying cause and repair/prevention of high temp concerning MDE raw water cooling system.

Prove proper operation of MDE cooling system to CG satisfaction.

Repair fuel oil leaks around the fuel oil pump on both ship service boilers, and prove proper operation of boiler safeties prior to placing the boilers in service.

Exhaust lagging around the turbochargers on both ships service generators is loose and of the wrong type. Install oil resistant lagging.

The rudder angle indicator in the SIE steering compartment is not working. Make repairs and prove proper operation.

Replace the burned out lamps for the engine shutdown overrides in the NYE pilot house

Prove proper operation of the OWS prior to placing in service.

Make permanent repairs to the #4 main propulsion motor damaged by a burnt transformer.

Prove proper operation of the #4 main propulsion motor in the presence of a Coast Guard Marine Inspector.

Prove proper operation of the propulsion system IAW approved PST procedures (section C).

Prove proper operation of the power generation and distribution system IAW approved PST procedures (section E).

Provide a technicians report as to the cause of the propulsion failure.

#2 SSDG diode pack failed and tripped the generator off line. Make repairs and prove proper operation of the #2 SSDG.

#1 SSDG start air pilot valve failed. Make repairs and prove proper operation of the #1 SSDG.

Provide containment around the fuel oil tank vents as required by 33CFR155.320(a)

Amend PST F-801 thru F-804 for the talk back system to reflect the test being conducted from the pilot house versus the MCR.

Make corrections to the fire control plan as discussed.

Prove proper operation of the elevators during the activation of the fire detection equipment, IAW PST line items A-143 & A-144.

Prove proper operation of the talk back system from the SIE wheelhouse.

Provide repair proposal for damaged hull plating and damaged frames. Proposal to include weld procedure qualifications & weld performance qualifications IAW 46CFR 57.03-.05 Repair proposal to be submitted and approved prior to commencing work.

Once repair propasal has been excepted by USCG. Make permanent repairs to hull and damaged frame IAW with excepted proposal. Prior to departure or carring passengers.

Make proper repairs to Ship Service Generator prior to departing port in Detroit, MI to the satisfaction of the attending Marine Inspector.

Schedule and complete "new to zone" inspection with USCG Activities New York prior to carrying parrsengers.
