A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. While underway, the vessel experienced a loss of propulsion and power to the port main engine. Vessel shall provide a technician's report attesting to the proper operation of the port engine and submit to the OCMI for review. 46 CFR 71.40-01

Each marine shipboard cable shall meet the requirements of IEEC, UL 1309 or IEC. Power cables for radars are UL E217996 extension cord for temporary installation.

Each Class 3 WTD must be tested IAW ASTM F 1196 / 1197. Both WTD's have Low pressure/ Low level indicator lights illuminated and doors did not close when initiated from the bridge control panel.

Each electrical connection must be made within an enclosure. Deck flood light splices were out in the open and taped, splice must be in an enclosure.

There must be ground detection for ships power system. Two of the ground detector lamps on the emergency switchboard were burned out.

Bulkheads of the “A” class shall be capable of preventing the passage of smoke and flame for 1 hour. Cable was improperly run through the Multi Cable Transit (Rox Blocks) between the CO2 room, generator room and emergency generator.

Inspector not limited from making such inspections to be assured of the safety of the vessel. Bulbs for status of the bow thruster control panel on the bridge burnt out. 21JUL-Update below corrected. Bulbs above still outstanding. Stbd SSDG oil psi gage for testing the low lube oil alarm/shutdown was inoperable.

Spaces protected by CO2 shall have warning signs. No warning sign posted the ECR door and E-GEN Space.

The installation of all systems of an electrical engineering or interior communications nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of sub chapter J (Electrical Engineering) of this chapter. When the potential of the final emergency power source reaches 85 to 95 percent of normal value, the emergency loads under § 112.15–5 must transfer automatically to the final emergency power source and, on a passenger vessel, this transfer must be accomplished in no more than 45 seconds after failure of the normal source of power. MI found emergency generator to not start after power failure. Item 01 Cleared via Tech Report today.

The fire main system must be operated and the pressure checked at the most remote and highest outlets. MI found fire pump in E/R with water leaking from mechanical seal. Seal requires replacement.

The respective piping systems installed on ships and barges shall have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, strainers, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Port & Starboard sea chest strainers were excessively clogged with debris. Verify strainer condition on the forward sea chest as well. Cite: 46 CFR 56.01-1(b)

The piping from the cylinders to the stop valves in the manifold shall be subjected to a pressure of 1,000 p.s.i. With no additional gas being introduced to the system, it shall be demonstrated that the leakage of the system is such as not to permit a pressure drop of more than 150 p.s.i. per minute for a 2-minute period. Upon inspection MI observed multiple loose hoses to CO2 bottle connections allowing un-threading by hand. All piping, valves, and fittings of ferrous materials shall be protected inside and outside against corrosion unless specifically approved otherwise by the Commandant. Upon inspection MI observed 02 fouled engine room CO2 discharge orifices. Captain or Engineer is required to provide a written statement of witnessing the repairs of both noted deficiencies pertaining to the CO2 system, in addition to, providing the updated re-servicing report from a qualified servicing contractor.

46 CFR 112.50-1(d) The generator set must be capable of carrying its full rated load within 45 seconds after cranking is started with the intake air, room ambient temperature, and starting equipment at O°C. The generator's prime mover must not have a starting aid to meet this requirement, except that a thermostatically-controlled electric water-jacket heater connected to the final emergency bus is permitted. The EDG failed to come on line supplying power within the required time. It would not have come on either until it was found that the voltage regulator was left in the off position for possibly a day and a half of operations of the vessel. Deficiency corrected by placing the regulator in the Auto position.

Currently the vessels bow thruster is not operational and has electrical issues. Prior to placing the vessel back into service make proper repairs/adjustments to the bow thruster and provide a detailed tech. report.

There shall be no accumulation of oil which might create a fire hazard in the bilges of machinery spaces. Marine Inspector (MI) observed excess lube oil under Port and Starboard M/E and fuel oil under the Port Generator. Repair to satisfaction of attending MI. 46CFR71.25-45

Each feeder current for steering must be protected by a circuit breaker. MI observed Flanking/Steering Gear Pump #1 data plate states 150A, however 30A is currently utilized. Provide corrective action plan. 46CFR58.25-55(a)

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering compartment. During steering test, the MI observed 4 degrees variance in indicators on Port and 3 degrees on starboard at hard over; 3 degrees at amidships. Repair to satisfaction of attending MI.

Each vessel must be provided with a sound-powered telephone system. Sound-powered telephone observed to be inoperable in multiple spaces onboard. Repair to satisfaction of attending MI. 46CFR58.25-15

Doors for 'A' Class bulkheads shall meet the requirements of 46CFR72.05-25(b). MI observed the 'A' self-closing door located between the ECR and the CO2 protected generator room (machinery space)unable to self close when the generators were running. 46CFR72.05-25(b)

In the event of an accident to a boiler, unfired pressure vessel, or machinery tending to render the further use of the item unsafe until repairs are made, or if by ordinary wear such items become unsafe, a report shall be made by the chief engineer immediately to the Officer in Charge, Marine Inspection, or if at sea, immediately upon arrival at port. Vessel reported loss of Rudder.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Vessel's port engine activated a hi exhaust temp alarm causing a reduction in RPM on that engine. 46CFR 58.10-5 A technician's report attesting to the cause, the corrective action (s) and proper operation will be required to clear 835.

All installations shall be of marine type engines suitable for the intended service, designed and constructed in conformance with the requirements of this subchapter. Vessel port main engine had a reduction in RPM due to high temperature alarm. A technician's report attesting to the cause and the corrective action (s) and proper operation will be required to clear 835. 46CFR58.10-5

While underway, vessel experienced loss of propulsion and power to Port main engine. Vessel shall provide certified technician's report attesting to proper operation of port engine and marine inspector shall witness power trials prior to carriage of passengers. Vessel is authorized one time transit from Scarsdale Ferry Landing to Belle Chasse Ferry landing.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules. The vessel reportedly has a sheared starboard shaft. This must be corrected prior to carrying passengers. Provide a written root cause analysis for th shaft breakage, and proposal to check to port shaft for similar deficiencies. 46 CFR 58.05-1

The owner, agent, master, operator, or person in charge must, within 5 days, file a written report of any marine casualty required to be reported under 46 CFR 4.05-1. The relevant parties failed to report the marine casualty of shearing the starboard shaft of the Plaquemines pride. The relevant parties must submit the required CG-2692 form to the Coast Guard immediately. 46 CFR 4.05-10

In general, at least three reinspections shall be made on each vessel within one year. These reinspections will be made at approximately equal intervals between annual inspections. Due to extended Drydock and Shipyard period, vessel missed a quarterly exam, and the annual expires on 14 Feb 2021. Prior to the carriage of passengers and entering ferry service, the vessel must complete its annual inspection. Coordinate COI attendance through the cognizant OCMI. 46 CFR 71.30-1 46 CFR 71.01-10(b)

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States:Every power-driven vessel of 20 meters or over in length while navigating. Vessel did not provide bridge to bridge certification. Bridge to Bridge certification must be provided to the satisfaction of attending Coast Guard Marine Inspector. 47 cfr 80.1001

No means shall be provided for locking doors giving access to either of the 2 required means of escape, except that crash doors or locking devices, capable of being easily forced in an emergency, may be employed provided a permanent and conspicuous notice to this effect is attached to both sides of the door. This paragraph shall not apply to outside doors to deckhouses where such doors are locked by key only and such key is under the control of one of the vessel's officers. Quick acting watertight door in engineering control room going into aft engine-room has padlock ring. Padlock ring must be removed to the satisfaction of Coast Guard Marine Inspector. 46 cfr 72.10-20

Vessel reported loss of steering due to broken components. The main steering gear and rudder stock must be of adequate strength for and capable of steering the vessel at maximum ahead service speed, which must be demonstrated to the cognizant Officer in Charge, Marine Inspection.

Instructions for operating emergency steering gear on the main control station has the wrong pump number preventing the steering system to work properly in emergency mode in the case of a loss of steering. Procure new Emergency Steering Procedure placard with the correct steering pump number.

Hydraulic line fitting for watertight door going forward above the class 3 watertight door on the engine room has a small leak. Complete the necessary repairs to correct the leak.

A Small leak was found on coupling above the fuel tank for the emergency generator. Due to the leak there is fuel and grime residue on top of the tank. Correct leak on coupling and clean area affected by the leak.

A Jacket Water leak is present on the elbow going from the head to the exhaust manifold on the starboard generator. Complete the needed repairs to correct the deficiency.

Class 3 watertight door open close labeling leading to the thruster room is peeling off the bulkhead. Replace label to ensure proper instructions are provided for the operation of the door.

Escape hatch located in bow thruster room frozen in place.

Fire station #3 hose found leaking. Repair.

Lines of lifebuoys found excessively fraying. Replace.

Replace rotted cable leading to junction box on rescue boat.

Replace stbd navigation side light, found casing to be fractured with watrer ingress. Limited to daytime operation in good visibility until fixed.

Currently, both sliding watertight doors are only operable from the local hand pump. O/O shall repair the remote and local electric opening mechanisms for both class 3 WT doors prior to issuance of the COI

Provide NDT report of flanking rudder and main rudder arms where it connects to rudder post to determine if any stress cracks may exit. Providerepair proposal to any areas of concern from report provided. To the satisfaction of the attending Marine inspectoir.

Due to port engine failure, vessel rep to provide tech or divers report to USCG to determine cause of problem.

Make operable both sliding watertight doors.

Adjust Engine Room to Control Room watertight door to make watertight.

Develope and submit to the OCMI or MSC Automation Test Procedures in accordancewith 46 CFR 62.

Make repairs to the Bow Thruster/Fire Monitor motor and demonstrate proper operation of both to the attending Marine Inspector.

Make operable the emergency generator to assume the electrical load automatically.

Replace all main deck fire doors.

Replace the Emergency Generator fuel supply hose.

Replace the flexible hose on the fixed CO2 storage bottle near the time delay bottles.

Renew batteries for the emergency lights in the EDG Room and the passenger space.

Adjust Control Room door to make watertight.

Clean and monitor generator exhaust leak

Provide a means of protection or guard for all engine room and generator room expansion and cooling water tank site glasses.

Replace worn line on all liferings.

Relabel the forward and bow lifebuoys.

Prove operation of stabd aft most self-igniting light.

Provide a new latch for the aft starboard container and provide a means of seperation for childrens lifejackets.

Repair leaks in both stbd cooling water day tanks in engine room.

Remove excessive oil from under both MDE's

Remove oil from f/o transfer station sump in engine rm.

Repair/Replace shaft tachometer on starboard operating station.

Stbd aft lifejacket box - NLT 15FEB08, ensure box is properly marked with # of child-size lifejackets and contains no standing water.

Submit satisfactory servicing report of fixed CO2 system.

Clean the bilge in the bow thruster room.

Re-install protective grill to port generator's engine.

Provide approved automation test procedure and FEMA and demonstrate same.

Provide for life folats: paddles, water lights, sea painters, and weak links.

Provide garbage placard & waste management plan.

Provide emergency broadcast placard.

Post piping diagrams IWO bilge, ballast, fire main, and fuel oil transfer manifolds.

Provide letter from ABS dropping class.

Provide to Marine Safety Office New Orleans a complete set of drawings identical to those submitted to the Marine Safety Center via the Electronic Commerce Program IAW MSC Technical Note 01-98 dated 6/24/98.

Conduct fire, man overboard, and security drills in the presence of and to the satisfaction of the attending CG Marine Inspector.

Provide additional chain for anchor to increase scope and prove anchor can hold vessel fast.

Properly label the adult and child-size personal floatation devices (PFD) storage lockers in accordance with 46 CFR 199.70 (b)(2)(iii).
