DOUBLE JACKETED FUEL SUPPLY LINE FOR M/E CYLINDER #11 WAS FOUND WRAPPED WITH INSULATION TAPE AND CYLINDER #1 WAS FRAYED WHICH EXPOSED THE HIGH-PRESSURE FUEL LINE.
