A person may not employ or engage an individual in a position required to hold an MMC unless that individual maintains a current medical certificate. The vessels captain did not posses a current medical certificate.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Vessel paper charts not updated and electronic charts had no tracking of current edition.

A vessel on an oceans, coastwise, limited coastwise, or Great Lakes route must carry six hand red flare distress signals [...] and six hand orange smoke distress signals. Vessel had 3 hand red flare and 3 hand orange smoke IAW lakes, bays, and sounds. Vessel route will be restricted to lakes, bays, and sounds until rectified.

Each rescue boat and ring life buoy must be marked with Type II retroreflective material [...]. The Arrangement of the of the retroreflective material applied after March 11, 1996, must be as specified by IMO Resolution A.658(16): "Retro-reflective materials of sufficient width (5cm) should be applied around or on both sides of the body of the lifebuoy at four evenly-spaced points."

47 CFR 80.159(e)(1); Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: Where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. Operator permits were not available for inspection.

When, in accordance with § 178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. No stability letter was posted.

MMSI number programmed into the vessels DSC radio is not the MMSI number from the current FCC Station License. Program DSC radio with vessels assigned MMSI number from the FCC Station License. 46 CFR 184.502 47 CFR 80.103

DSC equipped radio is not receiving position data from installed navigation equipment. FCC regulations require this connection if navigational equipment is installed. Properly connect DSC radio to installed GPS. 46 CFR 184.502 47 CFR 80.1085(c)

Expired IBA on vessel waiting to be serviced and inspected. Until IBA is properly serviced and validated, vessel is restricted in operating no greater than one mile from shore.

Retro-reflective tape on three ring buoys not applied around or on both sides of the body of the lifebuoy at four evenly-spaced points. Apply Type II retro-reflective tape that meets the minimum technical specifications as laid out in IMO Resolution A.658(16) at four evenly-spaced points.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Discovered a bulkhead penetration in the #4 watertight bulkhead that is leaking and deteriorated due to corrosion from dissimilar metals. 46 CFR 182.100

02108 - Conductors must be protected in accordance with their current carrying capacities. If the allowable current carrying capacity does not correspond to a standard device size, the next larger overcurrent device may be used provided it does not exceed 150 percent of the conductor current carrying capacity. Installed overcurrent protection is greater than approved drawing permits. - 46 CFR 183.380(d) - 17.C

02108 - Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. / Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. 02115 - Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. Hole in bulkhead between port forward void and tank room, center top. - 46 CFR 179.320(a) - 16.C

02106 - Except for engine exhausts, each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 150 millimeters (6 inches) above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails. A positive action valve or cock that is located as close as possible to the hull is an acceptable means for complying with [the above]. Hull penetrations with plastic caps and check valves. - 46 CFR 179.350 - 16.C

03110 - A hatch exposed to the weather must be watertight, except that the following hatches may be weathertight: (1) A hatch on a watertight trunk that extends at least 305 millimeters (12 inches) above the weather deck; (2) A hatch in a cabin top; and (3) A hatch on a vessel that operates only on protected waters. Stern trim void hatches not watertight. - 46 CFR 179.360(a) - 50.C

02101 - A weathertight door must be provided for each opening located in a deck house or companionway. Weathertight door gaskets to main engine rooms deteriorated at base. – 46 CFR 179.360(d) – 50.C

09106 - Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Modifications to sewage system with several pipes cut and left open. - 46 CFR 176.700(a) - 50.C

13101 - Pipes used for wet exhaust lines must be Schedule 80 or corrosion-resistant material and adequately protected from mechanical damage. Port main engine exhaust cooling hose collapsed. - 46 CFR 182.430(d) - 16.C

13102 - Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Starboard generator exhaust cooling hose cracked due to excessive unsupported bending. - 46 CFR 182.430(b) – 16.C

11131 - Placards containing instructions for the donning and use of the life jackets aboard the vessel must be posted in conspicuous places that are regularly accessible and visible to the crew and passengers. Life jacket donning placards do not match type onboard. - 46 CFR 185.516(a) - 16.C

13104 - Each fixed power bilge pump must be self priming. It may be driven off the main engine or other source of power. It must be permanently connected to the bilge manifold and may also be connected to the fire main. If of sufficient capacity, a power bilge pump may also serve as a fire pump. Starboard fixed power bilge pump would not take suction. - 46 CFR 182.520(c) - 10.C

Repair cracked weld in Port generator sea water intake through-hull valve flange prior to carrying passengers, to the satisfaction of attending marine inspectors. 46CFR176.804(g)

Pin hole found in port catameran on the inboard side approximately 2 inches above waterline. Hole located in same space as fuel tank. 46CFR176.700

Vessel is limited to 30 knots when significant wave height is greater then 1.5 ft. This restriction will remain in effect until the vessel is no longer on RICH PASSAGE 1 run, approx 2 weeks.

Pre-Engineered system failed to automatically shut down engines.

Vee out and reweld cracked weld on portside jet drive transom IAW 46CFR177.100.

Vessel will be moving remote fuel shut off valves IAW 46CFR182.455(b)(4).

Replace cracked fuel oil filter casing, located on port side Main Engine prior to carrying passengers. 46 CFR 182.455(b)(5).

COMPLETE PERMANENT REPAIRS TO THE COOLING SYSTEM ON THE FORWARD STARBOARD ENGINE.

Replace in kind port auxiliary generator and prove proper operation to satisfaction of the attending Marine Inspector.

Clean oil out of bilges to reduce fire hazard, IAW 46 CFR 182.100. This must be done prior to the carriage of passengers.

Repair visual indicator for bilge high water alarm for port tank space IAW 46 CFR 182.530(c).

Provide CPR cards for 50% of the crew

Ensure all loudspeakers that are part of the public address system are functional.

Provide servicing certificates for all liferafts.

Two separate sections of hull plating have over 25% wastage in places. Crop and renew.

Perform underway Man overboard and fire drills prior to operation with passengers onboard IAW 46 CFR 185.520.

Provide proof of proper IBA servicing prior to operation with passengers onboard IAW 46 CFR 185.730.

Prove proper installation of fuel shutoff handles in main fuel tank space IAW 46 CFR 182.455 (b)(4).

Provide firefighting service certificate to show proof of fireboys being serviced

Vsl prohibited from operating until new to zone inspection is conducted.

Replace/repair stbd fwd R/W pump and demonstrate proper operation.

Provide service certificates.

Provide logs for training and dirlls.

Provide valid CPR/First-aid card.

Vessel limited to 150 total persons on board to comply with D17 100% primary lifesaving requirement until operator provides Sector Juneau with exemption request letter to allow for 152 total persons.

Vessel required to provide proof of current NOAA registration of EPIRB prior to the carriage of passengers.

Resolve discrepancy between stability letter drafts and freeboards, and actual applied draft marks (draft markings appear in error); apply amidships minimum-freeboard markings (i.e. loading marks) as a temporary guideline prior to carrying passengers.
