1030-Steering Gear alarm- Hydraulic power-operated steering gear shall be provided with a low-level alarm for each hydraulic fluid reservoir to give the earliest indication of hydraulic fluid leak. Audible and visual alarms shall be given on the navigation bridge and machinery space. When tested no audible or visual alarm was noted on the Navigation Bridge. 74SOLAS(04)II-1/29.12.2-10c

1685-Radio Comms-At least (03) 2-way VHF radiotelephone apparatus shall be provided on every cargo ship 500 gross tons or more. PSCO observed all 2-way VHF radios inadequately charged due to charging dock being unplugged. PSCO needed to test radios on emergency batteries to verify operation. Captain declares that battery dock was unplugged for maintenance. No records produced.74SOLAS(am) III/6.

1795 - PSCO observed oily residue in soot collection tank. Subsequent investigation led to removal of drain pipe and eductor which had heavy concentrations of oil. The drain pipe and eductor discharge directly overboard via fresh water overboard piping and does not process through oily water separator or 15 ppm Oil Content Meter. MARPOL 2011 Reg 15.1 30abc
