Whenever damage or deterioration to hull plating or structural members that may affect the seaworthiness of a vessel is discovered or suspected, the cognizant OCMI may conduct an internal structural examination... and may require the vessel to be dry docked or taken out of service to assess the extent of the damage, and to effect permanent repairs. Vessel discovered a crack in the starboard pontoon outboard weld effecting watertight integrity. Provide repair plan proposal and make adequate repairs to the satisfaction of the attending MI. 46 CFR 176.600 (d)

The cognizant OCMI may require that a vessel and its equipment undergo any additional test or inspection deemed reasonable and necessary to determine that the vessel and its equipment are suitable for the service in which they are to be employed. MI observed inability to accurately and safely test tightness of the structure of damaged area. When testing weld repair on pontoon hull, a pressure of 2.2 PSI should be applied with a leak indicating solution. High pressure (200 PSI) gauge used to monitor pressure is not able to securely attach to the hull fitting or allow for accurate determination of internal pressure of hull. Provide low pressure gauge for testing of internal structures to the satisfaction of the MI. 46 CFR 176.840

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. Access to fuel tank/emergency fuel shutoffs is easily accessible to all passengers, creating an unsafe condition. Make arrangements to restrict access to crewmembers only. 46 CFR 176.830

A vessel not required to have a power driven fire pump by § 181.300 must have at least three 9.5 liter (21/2 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words “FIRE BUCKET” must be stenciled in a contrasting color on each bucket. One of the vessel’s fire buckets did not have labeling or a lanyard. 46 CFR 181.610

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. Adult and child life jackets are stored together without means of separation. 46 CFR 180.78

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel did not have FCC Station License or Safety Radiotelephony Certificate. 46 CFR 184.502

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible; U.S. Coast Pilot or similar publication; Coast Guard Light List; Tide tables; and Current tables. Vessel did not have any charts or nautical publications onboard. 46 CFR 184.420(a)

The marine inspector may require that an abandon ship or man overboard drill be held under simulated emergency conditions specified by the inspector. Abandon ship and man overboard drill could not be conducted due to the crew unavailability. 46 CFR 176.808(g)

The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Fire drill could not be conducted due to the crew unavailability. 46 CFR 185.524 (a)

The marine employer shall ensure that all individuals engaged or employed on board a vessel are fully indoctrinated in the requirements of this subpart. The marine employer did not have means to conduct drug and alcohol testing. 46 CFR 185.212 46 CFR 4.06-1 (e)

No vessel subject to inspection under 46 U.S.C. 3301 will be navigated unless it is under the direction and control of an individual who holds an appropriate license or officer endorsement on his or her MMC. Crew is not provided that holds an appropriate MMC 46 CFR 15.515(c)

Each ring life buoy on a vessel must be marked with type II retro reflective tape in accordance with 46 CFR 164.018. 46CFR185.604(h)i

Damage was discovered on stern port side pontoon. An approximate 6 inch crack in the weld will require repair. Make permanent repairs to the satisfaction of the attending Coast Guard Marine Inspector prior to carrying passengers.

Mount fire extinguisher with approved marine bracket located in the stern closet.

Install approved screw down post terminal battery in starboard life ring light.

Repair oil pressure alarm at operating station for port and starboard outboard engines.

Repair forward anchor light prior to getting underway after sunset or low visibility.

Ensure adequate quantity of type I life jackets are available for all passengers listed on COI.

Ensure proper marking of life jacket storage areas to reflect correct numbers.

Owner reported crack in weld on transom. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Make vessel available for inspection prior to carrying passengers to the satisfaction of the attending Coast Guard marine inspector.

Vessel owner owner failed to ensure drills were conducted with appropriate training for crew in the event of a fire for the year of 2021. The master shall conduct sufficient fire drills to make sure that each crew member is familiar with his or her duties in case of a fire. Fire fighting drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. The owner and master of vessel shall demonstrate knowledge of 46 CFR 185.524, 46 CFR 185.420 and 46 CFR 520 and produce logbook or form that clearly identifies the requirements of documenting drills. Vessel shall not carry passengers. Vessel crew shall demonstrate a successful fire drill prior to departure from port to the satisfaction of the attending Coast Guard marine inspector.

essel Certificate of Documentation has expired. A Certificate of Documentation is required for the operation of a vessel in certain trades, serves as evidence of vessel nationality, and permits a vessel to be subject to preferred mortgages. Renew certificate of documentation prior to departure to the satisfaction of the attending Coast Guard marine inspector.

Vessel does not have current certificate of inspection on board. A vessel to which this subchapter applies may not be operated without having on board a valid U.S. Coast Guard Certificate of Inspection. Place Certificate of Inspection onboard vessel to the satisfaction of the attending Coast Guard marine inspector.

It was identified the [name withheld] stern is riding several inches lower in the water than previously observed and the watertight integrity of the port and starboard pontoons are in question. Owner acknowledged the pontoons may have water ingress. Whenever damage or deterioration to the hull plating or structural members that may affect the seaworthinesses of a vessel is discovered or suspected the cognizant OCMI may conduct an internal structure exam in any affected space including fuel tanks, and may require the vessel to be dry docked or taken out of service to assess the extent of damage and to effect permanent repairs. Owner is to determine if pontoons are compromised and if taking on water and take satisfactory steps to prevent further water ingress and make vessel available for inspection prior to departure to the satisfaction of the attending Coast Guard Marine inspector.

Each Child life size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets.

All vessels must have navigation lights that are in compliance with applicable sections of the Inland navigation rules. Install forward mast light (white).

Electrical equipment used in a machinery spaces or exposed to corrosive environments must be protected. Find and repair prevent water egress into battery/electrical compartment.

01 - Replace 19 non-serviceable lifejackets with 19 serviceable lifejackets of approved type. Replaced 05 lifejackets. Reduce passenger count to 36. - 46CFR180.71(a)

Repair cracks on vessel transom.

Provide evidence of required servicing of vessel's portable fire extinguishers. 46CFR176.810(b)(1)

Provide current company/vessel employee list. 46CFR16.260

Provide approved type ring buoy lifeline of at least 60 feet. 46CFR180.70(c)

An annual inspection was not completed within intervals specified by 46CFR176.500(b)(1) and COI is no longer valid.

Fire in port outboard engine. Battery cable burnt from the outboard to the selector switch. Most likely cause is battery cable bracket became loose on the engine causing cable to shift up and rub against throttle linkage.
