A vessel not required to have a power driven fire pump by 181.300 must have at least three 9.5 liter (2.5 gallon) buckets, with an attached lanyard satisfactory to the cognizant OCMI, placed so as to be easily available during an emergency. The words "FIRE BUCKET" must be stenciled in a contrasting color to each bucket. Vessel has zero fire buckets on board. Attain and properly stow required fire buckets to the satisfaction of the attending CG marine inspector. 46 CFR 181.610

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant Officer in Charge, Marine Inspection, but must not be fewer than the minimum number required by Table 181.500(b). Space: Operating Station, Minimum Required Rating: 10-B:C. The operating station did not have a 10-B:C portable fire extinguisher properly mounted. Properly mount required extinguisher to the satisfaction of the attending CG marine inspector. 46 CFR 181.500(a)

Except as otherwise provided in this section every vessel must be fitted with a suitable magnetic compass designed for marine use, to be mounted at the primary operating station. Vessel does not have a magnetic compass on board. Obtain and mount required magnetic compass at the primary operating station to the satisfaction of the attending CG marine inspector. 46 CFR 184.402(a)

Each ship transporting more than six passengers for hire equipped with a radiotelephone station in accordance with Part III of Title III of the Communications Act must carry a radio operator who meets the following requirements: where the station power does not exceed 250 watts carrier power or 1500 watts peak envelope power, the radio operator must hold a marine radio operator permit or higher class license. None of the crew members present had their marine radio operator permit on board. Provide a marine radio operator permit for a crew member to the satisfaction of the attending CG marine inspector. 47 CFR 80.159(e)

Ring life buoy must be marked with Type II retroreflective material. Life ring did not have retroreflective material. 46 CFR 185.604(i)

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with §80.59 once every five years. Vessel does not have a valid Safety Radiotelephony Certificate. Vessel shall not operate more than 1,000 feet from shore without a valid Safety Radiotelephony Certificate. 47 CFR 80.903

Exhaust blower switches must be of the type interlocked with the starting switch and the ignition switch so that the blowers are started before the engine starter motor circuit or the engine ignition is energized. A red warning sign at the switch must state that the blowers must be operated prior to starting the engines for the time sufficient to insure at least one complete change of air in the space served. The exhaust blower switch was not interlocked with the engine starter circuit, and an appropriate red warning sign was not present. Post warning sign, and install blower IAW 46CFR182.460. 46CFR182.460(e)

Each battery must be secured to protect against shifting with the roll and pitch of the vessel. The port and starboard batteries were not secured. Secure the batteries to prvent against shifting. 46CFR183.350(b)

Each life jacket kept in a stowage container must be readily available. PFD stowage location was not labeled. Label the lifejacket stowage compartment with type (Adult/Child) and quantity of lifejacket. 46CFR180.78(a)(2)

The master and the crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. The vessel's emergency instructions were not posted. Post instructions at the operating station. 46CFR185.510

Alcohol testing of each individual must be conducted within 2 hours of when the Serious Marine Incident occurred, unless precluded by safety concerns directly related to the incident. The operator’s alcohol test strips were expired. Provide proof of valid alcohol test strips. 46CFR4.06-3(a)(i)

To enhance protection of life and property it is mandatory that each 406.0-406.1 MHz EPIRB be registered with NOAA before installation and that information be kept up-to-date. NOAA EPIRB registration was expired. Renew EPIRB registration. 47CFR80.1061(f)

A submersible electric bilge pump may be used as a power bilge pump required by Table 182.520(a) only on a vessel of not more than 19.8 meters (65 feet) in length carrying not more than 49 passengers provided that the pump is permanently mounted. Both electric bilge pumps under the operating station are not permanently mounted. Permanently affix both bilge pumps.

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found under center console. Terminate all exposed conductors.

When, in accordance with §178.210 of this chapter, a vessel must be provided with a stability letter, the stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station of the vessel. The vessel does not have a stability letter posted.

Stations in the maritime service must be licensed by the FCC either individually or by fleet. The vessel does not have a valid FCC Station License. Provide valid license. 47CFR80.13(a)

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio missing MMSI and LAT/LONG information. Input MMSI and LAT/LONG information into VHF radio. 47CFR80.905

Disposable hydrostatic release units are required to be replaced 2 years after the installation date. The provided hydrostatic release was expired. Provide a new hydrostatic release prior to operating the vessel with passengers along the Na Pali coast.

Vessel that have a FCC station license are required to have a valid Radiotelephony Safety certificate. This vessel does not have a valid certificate. Provide a valid FCC Safety radiotelephony ceritificate.

Spaces containging gasoline tanks are required to have installed/fixed ventillation systems with a mechanical exhaust blower. The space containing the gas tank does not have a mechanical blower installed. Provide a mechanical blower in the ventillation for this space.

Life jackets are required to be marked with the vessel's name/identification. New life jackets were provided for the vessel and not marked. Mark the life jackets with the vessel's name.

Ring buoys are required to me marked with the ships name and port. The provided ring buoy was not marked. Mark the vessels name and the hailing port on the ring buoy.
