III/1) and an engine rating (Reg. III/4) must be carried out in addition to the above." Every oil fuel pipe, which if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occuring in the space in which such tanks are situated. Upon examination of the remote fuel shutoff for the vessels emergency diesel generator, it was discovered to be seized to the deck rendering it inoperable in the case of fire or other emergency. 2550- The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant rules and regulations and with any additional requirements which may be established by the company. As per the Liberty one Shipmanagement "Instructions for Maintenance Work", The vessels emergency diesel generator and its quick closing valves are to be checked at weekly intervals by the chief engineer. PSCO discovered the quick closing valve on the emergency diesel generator to be seized to the deck rendering it inoperable in the event of an emergency. 2110-Oil/Oily mixtures Machinery-Oil filtering equipment shall be of design approved by Admin, ensure oil content discharge does not exceed 15 PPM. MEPC.60(33) system has discharge connection installed for port state control testing valve (one way) is not installed to prevent bypass. Vessel reported fluke on PORT anchor missing prior to entry into Port of Houston. Vessel was placed under OPCON requiring class to attest to condition found and place condition of class on vessel. Class has requested vessel to have tug escort while entering or departing port until repair is made.

Vessel does not have vessel specific Ballast Water Management Plan onboard.

The vessel wishes to enter the Port of New York with a hydraulic malfunction of its controllable pitch propeller that resulted in a loss of ship's propulsion.
