Each OSV carrying NLS in bulk in integral or fixed independent tanks must have a Cargo Record Book. Vessel did not have a Cargo Record Book for the carriage of methanol.

No OSV may carry flammable or combustible liquid cargoes without approval. In accordance with approval letter for the vessel's methanol arrangements (E1-0306569) dated 22SEP2003, the methanol tanks including the associated fittings shall comply with MVI Policy Letter No. 91-93, dated 30SEP1993 "Carriage of Methanol (Methyl Alcohol) on Offshore Supply Vessels". All tanks carrying methanol shall have either a restricted or closed gauging device. Vessel had only open gauging arrangements. The carriage of methanol is restricted until vessel is equipped with a restricted or close gauging device.

The opening below the lowest course of the guard rails shall not exceed 9 inches. Marine Inspectors (MIs) identified multiple freeing ports over a 9 inch opening. 46 CFR 127.310

Each OSV of less than 100 gross tons must be fitted with mooring and towing equipment meeting the applicable requirements for small passenger vessels in § 184.300 of this chapter. MIs identified the anchor seized at the waters edge upon testing. 46 CFR 130.250

All rotating machinery capable of injuring personnel must be provided with adequate covers or guards. MIs identified a pump missing a guard in the methanol room. 46 CFR 58.01-20

Each cable installation must meet Sections 6, of IEEE 45.8-2016. MIs identified a dead end cable in the methanol room. 46 CFR 111.60-5

Manholes and flush scuttles in position 1 or 2 or within superstructures other than enclosed superstructures shall be closed by substantial covers capable of being made watertight. Unless secured by closely spaced bolts,the covers shall be permanently attached. The engine room escape freeman hatch needs to be permanently attached. 46 CFR 42.15-40 (a)

The installation of diesel fuel piping must comply with the requirements of § 56.50-70(b). MIs identified a unsecured fuel line on the port main diesel engine.

Each vessel must have a permanently exhibited, for the guidance of the master and crew members, general arrangement plans showing, for each deck, the various fire-retardant bulkheads together with particulars of the: (a) Fire-detection systems; (b) Manual-alarm systems; (c) Fire-extinguishing systems; (d) Fire doors; (e) Means of ingress to the different compartments; and (f) Ventilating-systems, including the— (1) Positions of the dampers; (2) Site of the remote means of stopping the fans; and (3) Identification of the fans serving each section. The vessel’s fire plan needs to be updated and Coast Guard reviewed and approved. 46 CFR 131.945

Line-throwing gun equipment list required 4 lines. The vessel had two. Inspectors required the vessel to acquire the extra two cannisters. Vessel obtained the missing cannisters. Deficiency cleared during the inspection.

Vessel is required to have 12 rocket parachute flares and only had 10. Inspectors required 2 additional flares which the vessel reps produced prior to the conclusion of the inspection. Cleared deficiency on the spot.

A distribution panel in the engine room had blanks which were too small and were possible to allow someone to place a finger into the electrical void. Inspectors required properly sized blanks to preclude possible electrical hazard. Vessel rep accomplished this during the inspection.

The general alarm must operate on emergency power. During the emergency shutdown, the alarm did not sound. Vessel was required to rectify this by the marine inspectors. They procured an additional battery, connected it, and demonstrated it was able to operate on emergency power.

A rudder-angle indicator must be installed at the steering-control station in the pilot house. The starboard rudder angle indicator at the pilothouse and the local steering station on non-followup mode steering was behaving erratically. 46 CFR 130.130(h)

At least half of the OSV's lifebuoys must be fitted with a self-igniting light. The self-igniting light on the port bridge lifebuoy was not working. 46 CFR 133.70 (a)(4)(ii)

Inspections of lifesaving equipment shall be logged for records. Vessel was not logging EPIRB tests. 46 CFR 131.565(a) 46 CFR 131.545(a)(7)

Fire Nozzle was not maintaining the pressure necessary to be able to extinguish a fire. Fire nozzle must be either repaired or replaced while being properly maintained.

Call Sign for EPIRB was incorrect with the old call sign and not updated. The EPRIB registration must be updated with the correct call sign.

The Machinery Escape Hatch was not able to be secured watertight. Ensure Machinery Escape Hatch is able to be properly secured watertight.

Painter Line on the starboard side Life raft was not properly connected. Painter Line must be properly connected to the weak link.

Life buoy on the deck near the hatch was for training purposes only without USCG approval information. Life buoys must be approved and ready for immediate use.

Rescue platform was not able to be secured to the deck during rescue operations. Rescue platform must be able to be secured to the deck to prevent from falling overboard.

Vessel General Permit on board showed old drydock dates from the 2014. Vessel shall update the Vessel General Permit and maintain a current copy onboard. Cited 40 CFR 122.2.

Extension cords being used under bridge console in place of permanent wiring.

Hydrostatic release on port liferaft installed incorrectly

The bow thruster starting battery is overcharging and gassing off.

The standby vessel kit is short of lifejackets. The vessel may only standby for 110 persons until the proper number of lifejackets are in place.

The nylon line on 3 lifebuoys is frayed and in disrepair.

Proved proper operation of the #3 Starboard P/V valve prior to carriage of methanol.

Renew frayed line for the life buoy light on the stbd side.

Label all valves in engine room.

Place emergency lighting in engine room to point at emergency escape door.

Provide CG approved lifefloat painters for both lifefloats prior to sailing.

Properly mark the emergency escape hatch in the e/r.

Properly label keel cooler valves in engine room.

Post emergency steering gear changeover instructions at the aft steering station.

Renew control battery for starboard service generator.

Repair or replace starboard shaft tach in pilothouse.
