All machinery and equipment that is not in proper working order . . . must be removed; made safe through marking, tagging, or covering; or otherwise made unusable. Vessel's stern capstan was found to be disassembled with electrical wires disconnected and left exposed. The power to the capstan was turned off, but not locked out or tagged out in any way. Company policy requires electrical lockout before machinery is repaired. Switch was locked out while CG was onscene and vessel rep was made aware of the procedures company shore-side maintenance personnel failed to follow.

Ensure all required nautical publications are currently corrected, and if using electronic versions be sure to have redundant system in place or paper copies as back up.

United Steel has recently had a Man Overboard who nearly lost their life. A causal factor of that Marine Casualty was that the handrails had been removed, allowing the fall overboard. The company was alerted of the condition and reminded of the requirement. The company assured the Coast Guard that 100% of the vessel crews had been instructed on their policy. Marine Inspectors observed USS-1 underway with their handrails removed. The Company should conduct hands on training with each employee and formally document the training.
